目的：探讨早期康复联合针刺拮抗肌对痉挛性偏瘫患者肌张力和预后效果的影响。方法：以简单随机法,将100例中风后痉挛性偏瘫患者随机分成分2组，每组50例。对照组患者在治疗期间给予传统针刺联合常规早期康复锻炼，观察组则康复期联合拮抗肌针刺进行干预。4周为1疗程，治疗1个疗程后，评估患侧痉挛状态（用改良Ashworth的量表评分，MAS）、肢体运动功能（用Fugl Meyer运动功能评分法，FMA）、日常活动能力（采用改良Barthel指数，MBI）。结果：治疗后，观察组肢体痉挛程度MAS评分和肌力结果分别为（1.58±0.6）分和（372.7±21.6）N，均优于治疗前和治疗后对照组测定结果（P<0.05）；观察组治疗后FMA评分和MBI评分分别为（82.8±10.6）分和（83.7±5.6）分，优于对照组评分结果（P<0.05）。结论：针刺拮抗肌对患侧痉挛肢体进行针对性治疗，可通过神经反复刺激，改善患肢肌张，提高运动的协调性。

English Summary:

To observe the effect of early rehabilitation combined with acupuncture antagonistic muscle on muscular tension and its prognosis of patients with spastic hemiplegia. Methods:A total of 100 patients with spastic hemiplegia after stroke were randomly divided into two groups with 50 cases in each. The patients in the control group were given conventional acupuncture combined with routine early rehabilitation exercise, while patients in the observation group were treated with acupuncture antagonistic muscle combined with antagonistic acupuncture. Four weeks for 1 course. After 1 course of treatment, the spastic state(with modified Ashworth score, MAS), limb motor function(using Fugl Meyer motor function score, FMA), and daily activity(using modified Barthel index, MBI) were evaluated. Results:After treatment, the scores of MAS and muscle strength of the observation group were(1.58±0.6) and (372.7±21.6), superior to those of the control group and pre treatment(P<0.05). The FMA score and MBI score of the observation group were(82.8±10.6) and (83.7±5.6), which were better than those of the control group(P<0.05). Conclusion:Acupuncture on antagonistic muscle can improve the tension of the affected limb and enhance coordination of body movement.
