| 目的：探讨益气活血、通阳泄浊方在改善冠心病慢性心力衰竭伴颈动脉斑块患者血脂及心功能的应用价值。方法：选取2011年1月至2015年3月期间我院确诊治疗的冠心病慢性心力衰竭伴颈动脉斑块患者84例，依据随即分配原则分为西医组和中医组，每组42例，西医组患者给予常规西医治疗（如利尿剂、转换酶抑制剂、地黄类制剂等治疗），中医组患者在此基础上给予益气活血、通阳泄浊方治疗，所有患者采用半自动分析仪检测血脂水平，血脂指标包括：总胆固醇（TC）、三酰甘油（（TG）、低密度脂蛋白（LDL-D），及采用超声心电图检测心功能，心功能指标包括：左室射血分数（LVEF）、心输出量（CO）、每博输出量（SV），统计分析所有患者治疗疗效、治疗前后心功能及血脂水平变化。结果：中医组和西医组患者治疗有效率分别为80.95%、59.53%，前者明显高于后者，差异有统计学意义（P<0.05）；中医组患者血清中TC、TG、LDL-D水平明显低于西医组，差异有统计学意义（P<0.05）；中医组患者LVEF、CO、SV水平明显高于西医组，差异有统计学意义（P<0.05）。结论：益气活血、通阳泄浊方具有化疲通络、通气益血之功效，可有效提高冠心病慢性心力衰竭伴颈动脉斑块的治疗效果，有利于降低患者体内血脂水平和促进患者心功能恢复，值得临床作进一步推广。 |

| To discuss the application value of Yiqi Huoxue & Tongyang Xiezhuo Decoction in improving blood lipids and cardiac function of patients with carotid plaques due to chronic heart failure of coronary heart disease. Methods: Eighty-four patients with carotid plaques due to chronic heart failure of coronary heart disease were selected from January 2011 to March 2015 in the Hospitals. All patients were randomly divided into western medicine group and Chinese medicine group with 42 cases in each group. Patients in western medicine group was treated with conventional western medicine (such as diuretics, converting enzyme inhibitors, rehmannia agents), while Chinese medicine group of patients additionally received Yiqi Huoxue & Tongyang Xiezhuo Decoction treatment. Semi-automatic analyzer was used to test blood lipid indexes including total cholesterol (TC), triglycerides (TG), and low density lipoprotein (LDL)-D). Ultrasonic electrocardiogram was used to test cardiac function indexes including left ventricular ejection fraction (LVEF), cardiac output (CO), and each output (SV). Curative effects and levels of heart function and blood lipid before and after treatment of all patients were analyzed. Results: Treatment efficiency of the Chinese medicine and the western medicine group were 80.95% and 59.53%, respectively. The former was obviously higher than that of the latter, showing the statistically significant difference (P<0.05). Serum TC, TG, LDL-D levels of the Chinese medicine group was significantly lower than that of the western medicine group (P<0.05). LVEF, CO and SV level of the Chinese medicine group was obviously higher than those of the western medicine group (P<0.05). Conclusion: Yiqi Huoxue & Tongyang Xiezhuo Decoction have the function of soothing the collaterals and nourishing qi and may effectively improve the therapeutic effect of carotid plaques due to chronic heart failure of coronary heart disease. It is helpful for reducing the patient′s body lipid levels and promotes patients cardiac function recovery, which is worth of further clinical promotion. |
