| 目的：研究急性脑梗死患者采用自拟活血化瘀方治疗后血液流变学及血脂水平的变化，探索该药的疗效及作用机制。方法：选取2016年6月至2017年6月鄂东医疗集团黄石市中心医院收治的急性脑梗死患者90例，随机分成对照组和观察组，每组45例，2组均采用西药阿托伐他汀钙片等基础治疗，观察组再此基础上加用自拟活血化瘀方，2组均治疗14 d后，观察血液流变学、血脂水平，并比较2组疗效。结果：2组患者治疗前血黏度比较，差异无统计学意义（P>0.05），观察组治疗后血黏度指标显著低于对照组（P<0.05）；观察组治疗后红细胞变形指数、红细胞聚集指数、血细胞比容、电泳指数及红细胞沉降率K值均有明显改善（P<0.05）；而对照组仅红细胞变形能力有明显改善（P<0.05），观察组改善优于对照组（P<0.05）；治疗前2组胆固醇（TC）、三酰甘油（TG）、低密度脂蛋白（LDL）比较，差异无统计学意义(P>0.05)，治疗后，2组均较治疗前下降，但观察组下降更明显（P<0.05），而HDL均较治疗前升高，观察组升高较对照组明显（P<0.05）；治疗后观察组有效率86.67％，对照组有效率68.89％，差异有统计学意义（P<0.05）。结论：自拟活血化瘀方治疗后，急性脑梗死患者疗效显著，且其高黏滞状态等血液流变学指标和血脂水平得到明显改善，值得临床推广。 |

| To investigate the changes of hemorheology and blood lipid levels in patients with acute cerebral infarction treated by self-made Huoxue Huayu Decoction，and to explore the efficacy and mechanism of this formula.Methods:Forty-nine patients with acute cerebral infarction were enrolled in our hospital from June to June 2017.They were randomly divided into control group and observation group.Forty-five patients were treated with western medicine atorvastatin calcium tablets.The observation group was added self-made Huoxue Huayu decoction.The two groups were treated for 14 days.The hemorheology，blood lipid levels were observed，and the efficacy of two groups were compared.Results:1) There was no significant difference in blood viscosity between the two groups (P>0.05).The blood viscosity index of the observation group was significantly lower than that of the control group (P<0.05); 2) the erythrocyte deformation index，the erythrocyte aggregation index，hematocrit，electrophoretic index and k value of erythrocyte sedimentation rate of observation group were significantly improved (P<0.05)，while the erythrocyte deformability in the control group was significantly improved (P<0.05).Compared with the control group，the above results of observation group were improved compared with those in the control group (P<0.05)；3) There was no significant difference in the levels of TC，TG and LDL between the two groups before treatment (P>0.05).After treatment，the above indexes were significantly higher than those before treatment (P<0.05)，and HDL was higher than that before treatment.The observation group was significantly higher than the control group (P<0.05); 4) after treatment group，the effective rate of observation group was 86.67%，and 68.89% in the control group.The difference was statistically significant (P<0.05).Conclusion:The patients with acute cerebral infarction have significant curative effect after treatment with Huoxue Huayu Decoction，and their hemorheological indexes and blood lipid levels have been improved significantly，which is worthy of clinical application. |
