| 目的：观察黄连温胆汤加减治疗老年胸痹症（冠心病心绞痛）的临床疗效及其对血液流变学、血管内皮功能、心功能及血脂水平的影响。方法：选取2015年6月至2018年5月蚌埠医学院第三附属医院收治的冠心病心绞痛患者132例作为研究对象，按照随机数字表法分为对照组和观察组，每组66例。对照组给予西药常规治疗，观察组在对照组治疗的基础上加减黄连温胆汤，观察2组患者的临床疗效、血脂（TC、TG、LDL-C、HDL-C）、血液流变学指标、血管内皮功能（NO、ET-1）、血清炎性反应因子（hs-CRP、TNF-α和IL-6)、心脏功能参数变化情况。结果：治疗后与治疗前比较，2组TC、TG、LDL-C、红细胞压积、全血黏度高切、全血黏度低切、血浆黏度明显降低，HDL-C水平明显升高（均P<0.05）；治疗后2组比较，观察组上述指标变化更明显，差异有统计学意义（P<0.05）。治疗后，2组ET-1、hs-CRP、TNF-α及IL-6水平明显降低，NO水平明显升高，观察组上述指标变化更明显，差异有统计学意义（P<0.05）。与治疗前比较，2组治疗后LVEDD明显降低（P<0.05），CO、CI、EF明显升高（P<0.05）；观察组治疗后LVEDD明显低于对照组治疗后水平（P<0.05），CO、CI、EF明显高于对照组治疗后水平（P<0.05）。治疗后，观察组总有效率（90.91%）明显高于对照组（77.27%），差异有统计学意义（P<0.05）。结论：黄连温胆汤治疗冠心病心绞痛具有降低血脂，改善血液流变学，降低血清炎性反应因子，保护血管内皮功能，改善心功能的作用，从而提高临床治疗效果。 |

| To observe the clinical effects of modified Huanglian Wendan Decoction in the treatment of coronary heart disease angina pectoris and the influence on the blood rheology，vascular endothelial function，heart function and the level of blood lipid.Methods:A total of 132 cases with coronary heart disease angina pectoris were divided into a control group and an observation group，with 66 cases in each group.The control group was given conventional western medicine treatment，while the observation group was treated with Huanglian Wendan Decoction based on the treatment of the control group.The clinical effect，the level of blood lipids(TC，TG，LDL-C，HDL-C)，the hemorheology index，the endothelial function(NO and ET-1)，the level of serum inflammatory cytokines(hs-CRP，TNF-α and IL-6)，the parameter of heart function of the 2 groups were observed.Results:After treatment，the level of TC，TG，LDL-C，red blood cells deposited，whole blood high shear viscosity，whole blood low shear viscosity，plasma viscosity of the 2 groups decreased obviously.The level of HDL-C was increased significantly，and the above indexes of the observation group changed more obviously.The differences were statistically significant(P<0.05).After treatment，the level of ET-1，hs-CRP，TNF-α and IL-6 of the 2 groups decreased obviously，and the level of NO increased significantly.The above indexes of the observation group changed more obviously，and the differences were statistically significant(P<0.05).Compared with before treatment，the LVEDD of the 2 groups decreased obviously，and the CO，CI，EF increased significantly.The LVEDD of the observation group after treatment was significantly lower than that of the control group after treatment(P<0.05).The CO，CI，EF were significantly higher than that of the control group after treatment(P<0.05).The total effective rate of the observation group after treatment(90.91%)were significantly higher than that of the control group(77.27%).The difference was statistically significant(P<0.05).Conclusion:Huanglian Wendan Decoction in the treatment of coronary heart disease angina pectoris can reduce the level of blood liquid，improve the blood rheology，protect endothelial function，and improve the cardiac function，so as to improve the clinical therapeutic effect. |
