| 目的：观察针药联合参麦注射液治疗突发性耳聋的临床疗效，分析中西医结合治疗机制，为临床治疗提供借鉴。方法：选取2016年3月至2017年7月十堰市太和医院耳鼻喉科收治的突发性耳聋患者224例作为研究对象，随机均分为对照组和观察组，每组112例，对照组给予抗炎、扩管、溶栓、营养神经等常规治疗，观察组在此治疗基础上循经取穴施针，辨证开药煎服，并静脉滴注参麦注射液。治疗15 d后统计耳鸣和眩晕症状消失平均时间（d），听力阈值、耳鸣、眩晕的愈显率和有效率，TCD监测治疗前后局部脑血流（rCBF）、眼肌前庭诱发肌源性电位(oVEMP)引出率及其相关参数P1和N1潜伏期、P1和N1振幅、P1-N1波间期，并比较2种治疗方案的临床疗效。结果：经15 d治疗，观察组oVEMP引出率、P1和N1振幅和CBF明显提高，P1和N1潜伏期、症状消失平均时间缩短、听力阈值降低，耳鸣、眩晕的愈显率和有效率提高，与对照组比较，差异有统计学意义（P<0.05）。结论：针刺联合参麦注射液这种中西医结合治疗可产生协同治疗作用，明显提高临床疗效，在治疗突发性耳聋时有一定的借鉴价值。 |

| To observe clinical efficacy of acupuncture combined with Shenmai Injection in the treatment to sudden deafness，and analyze the treatment mechanism of integrated Chinese and western medicine to provide reference for clinical treatment.Methods：A total of 224 cases of randomized patients in Taihe Hospital ENT Department from March 2016 to July 2017 who were admitted with sudden deafness were randomly divided into a control group and an observation group，with 112 cases in each group.The control group was treated by anti-inflammatory，expanding tubes，trophic nerve for normal treatments.On the basis of that，the observation group performed acupuncture at the acupoints along the meridian，taking decoctions after prescription of syndrome differentiation and intravenous drip of Shenmai Injection.After treatment for 15 days，the average disappearing time(d)of tinnitus and vertigo，hearing threshold，tinnitus and vertigo markedly effective rate and efficiency rate were calculated.TCD was used to monitor the regional cerebral blood flow(rCBF)，lead-out rate of ocular vestibular evoked myogenic potentials(oVEMP)before and after treatment and related parameters such as P1 and N1 latency，amplitude and wave interval of P1 and N1，thereby comparing the clinical efficacy of 2 treatment regimens.Results：After 15 d of treatment，in the observation group，the rate of oVEMP extraction，amplitude of P1 and N1，CBF were significantly increased.Incubation period of P1 and N1，the average time of symptoms' disappearing were shortened.The hearing threshold shortened，tinnitus，vertigo markedly effective rate and efficiency rate improved.Combined with the control group，the difference was statistically significant(P<0.05).Conclusion：In the course of treatment，acupuncture combined with Shenmai Injection could produce a synergistic therapeutic effect，significantly improving the clinical efficacy，and there is some reference value in the treatment to sudden deafness. |
