| 目的：探讨黄连素联合培坤丸治疗多囊卵巢综合征胰岛素抵抗（PCOS-IR）患者对脂糖代谢、炎性反应递质及激素水平的影响。方法：选取2016年3月至2017年3月南方医科大学附属小榄医院收治的PCOS-IR患者96例作为研究对象，按照随机数字表法将患者分为对照组与观察组，每组48例。对照组采用二甲双胍治疗，观察组采用黄连素联合培坤丸治疗。分析2组患者的临床疗效以及脂糖代谢、炎性反应递质、激素水平的变化。结果：观察组的治疗有效率为93.75%，与对照组的89.58%比较，差异无统计学意义（P>0.05）。治疗后2组患者的总胆固醇（TC），三酰甘油（TG）、低密度脂蛋白胆固醇（LDL-C）均低于治疗前，差异有统计学意义（P<0.05），而高密度脂蛋白胆固醇（HDL-C）无明显变化。治疗后观察组空腹胰岛素（FINS）、胰岛素抵抗指数（HOMA-IR）低于治疗前，对照组的空腹血糖（FBG）、HOMA-IR低于治疗前，差异有统计学意义（P<0.05）。治疗后2组患者血清C反应蛋白（CRP）、白细胞介素-6（IL-6）和肿瘤坏死因子-α（TNF-α）水平均低于治疗前，且观察组低于对照组，差异有统计学意义（P<0.05）。治疗前后2组患者的卵泡刺激素（FSH）比较，差异无统计学意义（P>0.05），治疗后2组患者的黄体生成素（LH）、睾酮（T）、LH/FSH均低于治疗前，且观察组LH、LH/FSH低于对照组，差异有统计学意义（P<0.05）。结论：黄连素联合培坤丸可有效治疗PCOS-IR，改善患者的脂糖代谢、炎性反应递质及激素水平，临床疗效与二甲双胍相当，但对炎性反应递质及激素水平的改善作用更为明显。 |

| To investigate the effects of berberine combined with Peikun pill on lipid and glucose metabolism，inflammatory factors and hormone levels in patients with insulin resistance of polycystic ovary syndrome(PCOS-IR).Methods:A total of 96 patients with PCOS-IR who were treated in Xiaolan Hospital Affiliated to Southern Medical University from March 2016 to March 2017 were selected as research subjects.The patients were divided into a control group and an observation group according to random number method，with 48 cases in each group.The control group was treated with metformin，and the observation group was treated with berberine combined with Peikun pills.The clinical efficacy，lipid and glucose metabolism，inflammatory factors and hormone levels of the 2 groups were analyzed.Results:The total effective rate of the observation group was 93.75%，which was not significantly different from the 89.58% of the control group(P>0.05).After treatment，the total cholesterol(TC)，triglyceride(TG)，low density lipoprotein cholesterol(LDL-C）of the 2 groups were all lower than those before treatment(P<0.05)，but high density lipoprotein cholesterol(HDL-C）had no significant change.After treatment，the fasting insulin(FINS）and insulin resistance index(HOMA-IR）of the observation group were lower than those before treatment，and the fasting blood glucose(FBG）and HOMA-IR of the control group were lower than before treatment(P<0.05).The levels of serum C-reactive protein(CRP)，interleukin-6(IL-6）and tumor necrosis factor-α(TNF-α）in both groups were lower than those before treatment，and the observation group were lower than those in the control group(P<0.05).There was no significant difference in follicle stimulating hormone(FSH）between the 2 groups before and after treatment(P>0.05).After treatment，the luteinizing hormone(LH)，testosterone(T）and LH/FSH of the 2 groups were all lower than those before treatment.The LH and LH/FSH in the observation group were lower than those in the control group(P<0.05).Conclusion:Berberine combined with Peikun pill can effectively treat PCOS-IR，improve lipid and glucose metabolism，inflammatory factors and hormone levels in patients.The clinical efficacy is comparable to metformin，but the improvement of inflammatory factors and hormone levels is more obvious. |
