| 目的：探讨鸦胆子油乳注射液联合放化疗对中晚期食管癌患者淋巴细胞CD4+、CD8+的影响。方法：选取2016年1月至2018年10月泉州海峡医院收治的食管癌患者45例作为研究对象，按照随机数字表法随机分为对照组（n＝22）和观察组（n＝23）。对照组给予同步放化疗治疗，观察组在此基础上给予鸦胆子油乳注射液辅助治疗，2组均连续治疗30 d。对2组患者治疗后临床疗效、不良反应情况及生命质量评分，以及2组患者治疗前后CD4+和CD8+及CD4+/CD8+变化差异进行分析。结果：治疗后，观察组、对照组总有效率分别为69.57%、40.91%，观察组高于对照组（P<0.05）；与治疗前比较，治疗后观察组CD4+、CD4+/CD8+均降低、CD8+升高，观察组低（高）于对照组，差异有统计学意义（P<0.05）；治疗后，观察组的生命质量评分的升高较对照组明显，差异有统计学意义（P<0.05）；治疗后，不良反应发生率观察组和对照组比较，差异无统计学意义（P>0.05），而骨髓抑制出现时间观察组长于对照组，差异有统计学意义（P<0.05）。结论：鸦胆子油乳注射液联合放化疗治疗中晚期食管癌可以改善临床疗效，减轻放化疗对中晚期食管癌患者免疫功能的损害，提高患者生命质量，且疗效优于单纯放化疗。 |

| To investigate the effects of Brucea javanica oil emulsion injection combined with radiotherapy and chemotherapy on CD4+，CD8+ lymphocyte in patients with advanced esophageal cancer.Methods:A total of 45 patients with esophageal cancer admitted to Quanzhou Strait Hospital from January 2016 to October 2018 were divided into a control group(n＝22)and an observation group(n＝23)according to random number table method.The control group was treated with concurrent radiotherapy and chemotherapy，while the observation group was treated with Brucea javanica oil emulsion injection as an adjuvant therapy.Both groups were treated continuously for 30 days.The clinical efficacy，adverse reactions and quality of life scores of the 2 groups after treatment，as well as the changes of CD4+ and CD8+ and CD4+/ CD8+ in the 2 groups before and after treatment were statistically analyzed.Results:After treatment，the total effective rates of the observation group and the control group were 69.57% and 40.91%，respectively.The observation group was higher than the control group(P<0.05).Compared with before treatment，the levels of CD4+，CD4+/CD8+ in the observation group decreased and CD8+ increased after treatment，and the levels in the observation group were significantly lower(higher)than those in the control group(P<0.01).After treatment，the quality of life score of the observation group was higher than that of the control group(P<0.05).After treatment，there was no difference in the incidence of adverse reactions between the observation group and the control group(P>0.05)，but the occurrence time of bone marrow suppression in the observation group was longer than that in the control group(P<0.05).Conclusion:Brucea javanica oil emulsion injection combined with radiotherapy and chemotherapy in the treatment of advanced esophageal cancer can improve the clinical efficacy，alleviate the damage of radiotherapy and chemotherapy to the immune function of patients with advanced esophageal cancer，improve the quality of life of patients，and the curative effect is better than that of radiotherapy and chemotherapy alone，which is worthy of clinical application. |
