目的：研究补肾益肺方联合沙美特罗替卡松治疗慢性阻塞性肺疾病（COPD）肺部感染患者的临床疗效；方法：选取2014年7月至2018年7月湖北省十堰市太和医院慢性病中心收治的COPD肺部感染患者138例作为研究对象，按照随机数字表法随机分为对照组和观察组，每组69例。对照组给予常规治疗+沙美特罗替卡松治疗。观察组给予常规治疗+沙美特罗替卡松+补肾益肺方治疗。观察2组患者治疗前后SaO2（动脉血氧饱和度）、PaO2（动脉血氧分压）、PaCO2（动脉血二氧化碳分压）等血气分析指标，FVC（用力肺活量）、FEV1等肺功能指标，血清IL-6（白细胞介素-6）、TNF-α（肿瘤坏死因子-α）、CRP（C-反应蛋白）等炎性因子，血浆MSP（巨噬细胞刺激蛋白）、血清淀粉样蛋白A（SAA）、触珠蛋白（HPT）、不良反应等指标。结果：治疗后，观察组SaO2、PaO2、FEV1、FVC等指标均高于对照组，PaCO2、IL-6、TNF-α、CRP、MSP、SAA、HPT低于对照组，差异均有统计学意义（P<0.05）。观察组总有效率（84.06%）高于对照组（63.77%），差异有统计学意义（P<0.05）。治疗期内，2组均无显著不良反应事件发生。结果：补肾益肺方联合沙美特罗替卡松治疗COPD肺部感染患者有明显疗效，可明显改善血气分析、肺功能、炎性因子及MSP、SAA、HPT等指标，疗效显著，安全可靠。

English Summary:

To study the clinical efficacy of Bushen Yifei Recipe combined with Salmeterol Fluticasone in the treatment of COPD patients with pulmonary infection.Methods:A total of 138 COPD patients with pulmonary infection admitted to our hospital from July 2014 to July 2018 were included in this study.They were divided into an observation group(n=69)and a control group(n=69)according to the random number table method.The control group was given conventional treatment and Salmeterol Fluticasone.The observation group was treated with conventional treatment, Salmeterol Fluticasone and Bushen Yifei Recipe.Before and after treatment, blood gas analysis indexes such as SaO2(Arterial oxygen saturation), PaO2(Arterial oxygen partial pressure), PaCO2(Arterial carbon dioxide partial pressure), pulmonary function indexes such as FVC(Forced vital capacity), FEV1, inflammatory factors such as serum IL-6(Interleukin-6), TNF-α(Tumor necrosis factor-α), CRP(C-reactive protein), Plasma MSP(Macrophage stimulating protein), Serum amyloid A(SAA), Haptoglobin(HPT), and adverse reactions were observed.Results:After treatment, SaO2, PaO2, FEV1 and FVC in the observation group were higher than those in the control group, while PaCO2, IL-6, TNF-α, CRP, MSP, SAA and HPT were lower than those in the control group(P<0.05).The total effective rate of the observation group(84.06%)was higher than that of the control group(63.77 %)(P<0.05).During the treatment period, no significant adverse events occurred in both groups.Conclusion:Bushen Yifei Recipe combined with Salmeterol and Fluticasone in the treatment of MOCD patients with lung infection can invigorate the kidney and benefit the lung, eliminate phlegm and asthma, remove inflammatory factors, improve blood gas analysis, lung function, inflammatory factors and MSP, SAA, HPT and other indicators, with obvious curative effect, safety and reliability.
