| 目的:探究清热止痛方联合穴位埋线治疗湿热瘀结型痛经效果,以期丰富治疗方法,提高疗效。方法:选取2014年7月至2018年6月秦皇岛市中医医院收治的湿热瘀结型痛经患者200例作为研究对象,按照就诊顺序编号随机分为对照组和观察组,每组100例。对照组常规西医治疗,观察组在对照组基础上加用清热止痛方联合穴位埋线治疗,同时治疗4周。观察2组患者治疗前、完成治疗后疼痛视觉评分(VAS评分)、COX痛经症状评分量表(CMSS)疼痛持续时间、中医症状评分、子宫动脉血流动力学、炎性反应、性激素变化并比较;完成治疗后进行疗效评价。结果:1)完成治疗后2组患者VAS评分、CMSS、疼痛持续时间、中医症状评分较治疗前均显著下降(P<0.05),完成治疗后观察组以上指标均显著低于对照组(P<0.05)。2)完成治疗后2组患者组胺(HIS)、前列腺素2α(PGF2α)、纤维蛋白原(Fib)、C反应蛋白(CRP)、白细胞介素-6(IL-6)较治疗前均显著下降(P<0.05),完成治疗后2组患者以上指标均显著低于对照组(P<0.05)。3)完成治疗后2组患者搏动指数、阻力指数、收缩期/舒张期峰值较治疗前均显著下降(P<0.05)。完成治疗后观察组以上指标显著低于对照组(P<0.05)。4)完成治疗后2组患者雌二醇(E2)较治疗前均显著下降,孕酮(P)显著升高(P<0.05),完成治疗后观察组E2显著低于对照组,P显著高于对照组(P<0.05)。5)完成治疗后观察组痊愈率、总有效率均显著高于对照组,差异有统计学意义(P<0.05)。结论:清热止痛方联合穴位埋线治疗湿热瘀结型痛经可改善子宫动脉血液循环,抑制炎性反应,提高痛阈,从而提高疗效。 |

| To explore the effects of Qingre Zhitong Decoction combined with catgut embedding at acupoints on dysmenorrhea of damp-heat stagnation type, in order to enrich the treatment methods and improve the curative effect.Methods:A total of 200 patients with dysmenorrhea of damp-heat stagnation type hospitalized in Qinhuangdao Chinese Medicine Hospital from July 2014 to June 2018 were randomly divided into a control group(100 cases)and an observation group(100 cases)according to the order of treatment.The control group was treated with routine western medicine, while the observation group was treated with Qingre Zhitong Decoction combined with acupoint catgut embedding for 4 weeks on the basis of the control group.The pain visual score(VAS), COX dysmenorrhea symptom scale(CMSS), pain duration, TCM symptom score, uterine artery hemodynamics, inflammation and sex hormone changes were observed and compared before and after treatment in the 2 groups.Results:After treatment, the VAS score, CMSS, pain duration and TCM symptom score of the 2 groups were significantly decreased than those before treatment(P<0.05), and the above indexes of the observation group were significantly lower than those of the control group(P<0.05).2)After treatment, the levels of histamine(HIS), prostaglandin 2A(PGF2a), fibrinogen(Fib), C-reactive protein(CRP), interleukin-6(IL-6)in the 2 groups were significantly decreased than those before treatment(P<0.05), and the above indexes in the 2 groups were significantly lower than those in the control group(P<0.05).3)After treatment, the pulsation index, resistance index and systolic/diastolic peak values of the 2 groups decreased significantly compared with those before treatment(P<0.05).After treatment, the above indicators in the observation group were significantly lower than those in the control group(P<0.05).4)After treatment, estradiol(E2)in both groups decreased significantly, progesterone(P)increased significantly(P<0.05).After treatment, E2 in the observation group was significantly lower than that in the control group, and P was significantly higher than that in the control group(P<0.05).5)After treatment, the recovery rate and total effective rate of the observation group were significantly higher than those of the control group, and the difference was statistically significant(P<0.05).Conclusion:Qingre Zhitong Decoction combined with acupoint catgut embedding therapy can improve uterine artery blood circulation, inhibit inflammation, increase pain threshold, and thus improve the curative effect. |
