目的:分析癌胚抗原(CEA)、甲胎蛋白(AFP)在肝癌诊断与鉴别中的应用价值。方法:选取2017年1月至2018年12月陕西中医药大学收治的肝癌患者为肝癌组(n=120)、肝硬化患者为肝硬化组(n=100)、同期健康体检者为对照组(n=50),测定CEA、AFP并进行比较,同时以病理结果为参照,对CEA、AFP、CEA+AFP敏感度、特异度、准确度进行对比。结果:3组CEA、AFP比较,肝癌组>肝硬化组>对照组,差异有统计学意义(P<0.05);肝癌组内Ⅱ期CEA、AFP<ⅢA期<ⅢB期+Ⅳ期,差异有统计学意义(P<0.05);肝癌组内肝细胞性肝癌CEA、AFP<胆管细胞性肝癌,差异有统计学意义(P<0.05);在肝癌诊断中CEA+AFP敏感度、特异度、准确度>CEA和AFP,差异有统计学意义(P<0.05),CEA、AFP差异无统计学意义(P>0.05)。结论:在肝癌诊断与鉴别中CEA、AFP均能够取得较为理想的应用价值,二者联合更有助于提高诊断效能。 |

To analyze the practical application value of carcinoembryonic antigen(CarcinoEmbryonic Antigen,CEA)and alpha-fetoprotein(AFP)in the diagnosis and identification of liver cancer.Methods:A total of 120 patients with liver cancer admitted to from January 2017 to December 2018 were enrolled in the liver cancer group,100 patients with cirrhosis were in the cirrhosis group,and 50 healthy subjects in the same period were used as the control group.CEA and AFP were measured and performed.The sensitivity,specificity and accuracy of CEA,AFP,CEA+AFP were compared.At the same time we refer to pathological results.Results:Compared with CEA and AFP of the 3 groups,liver cancer group>liver cirrhosis group>control group,the difference was statistically significant(P<0.05).In the liver cancer group,in the stage II CEA,AFPthe difference between CEA and AFP was statistically significant(P<0.05).There was no significant difference between CEA and AFP(P>0.05).Conclusion:CEA and AFP can achieve better practical application value in the diagnosis and identification of liver cancer.The combination of the 2 can help improve the diagnostic efficiency and is worthy of being promoted in clinical work in the future. |
