| 目的：系统评价近10年头针治疗PSD的疗效和安全性。方法：检索Pubmed、国家知识基础设施数据库（CNKI）、中国学术期刊数据库（CSPD）、中文科技期刊数据库（CCD）和Cochrane Library数据库，收集单用头针或以头针为主要治疗手段诊疗PSD的相应随机对照试验（RCT），并且追纳类似RCT的参考文献，以获取遗漏的相关资料，检索时限均为从2009年1月1日至2019年12月1日。采用循征医学的评价方法，对符合要求的RCT进行数据统计，采用Revman5.3软件进行Meta分析。结果：共纳入16篇RCT，共计1 342例患者，通过对照组干预措施的不同，分2组进行分析：1）头针与西药组；2）头针与常规针刺组。Meta分析结果显示：1）头针与西药治疗PSD有效率比较，合并后OR值为2.01，95%CI为1.32~3.06，P=0.001，且治疗后汉密尔顿抑郁量表(HAMD)评分结果合并后MD值为-5.43，95%CI为-6.01~-4.85，差异有统计学意义(P<0.000 01），表明头针治疗PSD疗效优于西药治疗。2）头针与常规针刺治疗PSD有效率比较，合并后OR值为2.52，95%CI为1.44~4.39，P=0.001，且治疗后HAMD评分结果合并后MD值为-4.17，95%CI为-4.47~-3.08，差异有统计学意义(P<0.000 01），发现头针治疗PSD疗效高于普通针刺治疗。结论：头针治疗PSD疗效可靠，但本Meta分析纳入文献较少，质量较低，仍需要高质量、大样本的RCT验证。 |

| To systematically evaluate the efficacy and safety of Post-stroke Depression(PSD) treated with acupuncture in the past 10 years.Methods:We searched Pubmed,National Knowledge Infrastructure Database(CNKI),China Academic Journals Database(CSPD),Chinese Sci-tech Journals Database(CCD) and Cochrane Library Databases,and collected the corresponding random trial of the diagnosis and treatment of PSD with single-use scalp acupuncture or scalp acupuncture as the main treatment method as the controlled Trials(RCT),and followed up references similar to RCTs to obtain missing relevant information.The retrieval time was from January 1,2009 to October 30,2019.The evaluation method of evidence-based medicine was adopted to make statistics on the RCT that met the requirements,and the Revman5.3 software was used for Meta analysis.Results:A total of 16 RCTS were included,with 1 342 patients in total.Through different intervention measures in the control group,analysis was conducted in 2 groups:1)head acupuncture and Western medicine group;2)head needle and conventional acupuncture group.Meta analysis results showed that:1)Comparison of effective rate between head acupuncture and Western medicine in the treatment of PSD,OR value after the combination was 2.01,95%CI：1.32 to 3.06,P=0.001,and after treatment,the combined MD value of HAMD score was-5.43,95%CI:-6.01 to -4.85,P<0.000 01.The results showed that the curative effect of PSD treated with head acupuncture was better than that of Western medicine,and the difference was statistically significant.2)After the Head acupuncture and conventional acupuncture treatment of PSD effective rate comparison,combined OR value was 2.52,95%CI:1.44 to 4.39,P=0.001,and after the treatment of HAMD scores combined MD value of 4.17,95%CI:-4.47 to -3.08,P<0.000 01.It was found that the efficacy of head acupuncture in the treatment PSD was higher than ordinary acupuncture treatment,the difference was statistically significant.Conclusion:The efficacy of head acupuncture in the treatment of PSD was reliable.However,this analysis included few literature and the quality was low,so high-quality and large-sample RCT are still needed for verification. |
