目的：探究针刺放血联合血府逐瘀汤治疗下肢静脉曲张的临床疗效。方法：选取2019年1月至2019年12月上海中医药大学附属曙光医院诊治的下肢静脉曲张患者65例作为研究对象，随机分为对照组（n=32）和观察组（n=33）。对照组采用治疗型静脉曲张袜加压治疗，观察组采用针刺放血联合血府逐瘀汤治疗。采用静脉临床严重程度评分表、生命质量调查表比较2组患者在治疗前、治疗1个月后、2个月后的下肢静脉情况和生命质量，观察并分析2组患者血液流变学相关指标的变化、血清免疫炎症介质的变化，比较2组患者的临床疗效。结果：与治疗前比较，2组患者治疗1个月、2个月后静脉临床严重程度评分均下降，且治疗后的相同时间点，观察组患者静脉曲张好转程度明显优于对照组（P<0.05）。与治疗前比较，2组患者治疗1个月、2个月后生命质量评分均有所提高，且观察组高于对照组（P<0.05）。治疗后，2组患者血液流变学相关指标均趋正常，且观察组流变学指标低于对照组（P<0.05）。治疗后，2组患者血清免疫炎症反应相关指标均较治疗前降低，且观察组低于对照组（P<0.05或P<0.01）。治疗后，观察组有效率（93.94%）显著高于对照组（75.00%）（P<0.05）。结论：针刺放血联合血府逐瘀汤治疗下肢静脉曲张疗效可靠，值得推广。

English Summary:

To explore the clinical efficacy of acupuncture and bloodletting combined with Xuefu Zhuyu Decoction in the treatment of lower extremity varicose veins.Methods:Sixty-five patients hospitalized for varicose veins of lower extremities in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 2019 to December 2019 were enrolled and divided into a control group(n=32) and an observation group(n=33).The patients in the control group were treated by compression socks,and those in the observation group were treated by acupuncture and bloodletting combined with Xuefu Zhuyu Decoction.The lower extremity veins and quality of life of patients in the two groups were compared before treatment,one month,and two months after treatment by the venous clinical severity score and quality of life questionnaire.The changes in blood rheology-related indexes and serum immunoinflammatory mediators of patients in the two groups were observed and analyzed.Additionally,clinical efficacies of the two groups were compared.Results:The venous clinical severity scores of patients in both groups decreased after one month and two months of treatment,and the observation group was superior to the control group in the improvement of varicose veins at the same time point after treatment(P<0.05).The quality of life scores of patients in both groups increased after one month and two months of treatment,and the observation group was higher than the control group(P<0.05).After treatment,blood rheology-related indexes in the two groups were prone to be restored,and the indexes in the observation group were lower than those in the control group P<0.05).After treatment,serum immunoinflammatory response-related indexes in the two groups were lower than those before treatment,and the decrease in the observation group was less significant than the control group(P<0.05 or P<0.01).After treatment,the effective rate of the observation group was 93.94%,higher than 75% in the control group(P<0.05).Conclusion:Acupuncture and bloodletting combined with Xuefu Zhuyu Decoction have a reliable curative effect on varicose veins of lower extremities,which are worthy of promotion.
