      目的:基于Louvain算法探讨生脉注射液治疗心力衰竭(HF)的优效方案。方法:对使用生脉注射液治疗HF患者的用药方案进行分析。从全国29家三级甲等医院的医院信息管理系统(HIS)数据库中选取3 382例使用生脉注射液后症状改善好转的HF患者住院信息,采用Louvain算法进行数据分析,探究生脉注射液治疗HF的优效治疗方案。结果:针对HF患者的基础治疗可使用“生脉注射液+阿司匹林+辛伐他汀+美托洛尔+卡托普利”的用药方案;针对水肿较为严重的患者,可使用“生脉注射液+地高辛+呋塞米+螺内酯+门冬氨酸镁”的用药方案;合并上呼吸道感染的患者,可使用“生脉注射液+头孢呋辛+左氧氟沙星”的用药方案;合并慢性阻塞性肺疾病的患者,可使用“生脉注射液+头孢哌酮和β-内酰胺酶抑制剂+茶碱+氨溴索+痰热清注射液”的用药方案”;合并糖尿病的患者,可使用“生脉注射液+胰岛素”的治疗方案;合并高血压的患者,可使用“生脉注射液+单硝酸异山梨酯+比索洛尔+丹红注射液”治疗方案;合并脑出血的患者,可使用“生脉注射液+甘露醇+醒脑静注射液”的方案。结论:本研究中所发现的优效方案与临床指南基本一致,但由于本研究是属于真实世界的研究,数据收集过程中会有一定的缺失,加之临床实际情况也较为复杂,所以提供的临床用药方案可供参考,目前仍需要大量的临床研究来探索更多疗效显著的联合用药方案。
English Summary:
      To analyze the optimal protocols of Shengmai Injection in the treatment of heart failure(HF) based on the Louvain algorithm.Methods:The medication protocols of Shengmai Injection in the treatment of HF were analyzed.The medical data of 3 382 inpatients with HF whose symptoms were improved after using Shengmai Injection were collected from the Hospital Information System(HIS) of 29 Grade III Level A hospitals in China.The data were analyzed by using the Louvain algorithm to explore the optimal protocols of Shengmai Injection against HF.Results:For the basic treatment of HF patients,Shengmai Injection combined with aspirin,simvastatin,metoprolol,and captopril could be used; for those with severe edema,Shengmai Injection combined with digoxin,furosemide,spironolactone,and potassium magnesium aspartate could be employed; for those with upper respiratory tract infection,Shengmai Injection combined with cefuroxime and levofloxacin could be used; for those with chronic obstructive pulmonary disease(COPD),Shengmai Injection combined with cefoperazone and β-lactamase inhibitor,theophylline,ambroxol,and Tanreqing Injection could be used; for those with diabetes,Shengmai injection combined with insulin could be used; for those with hypertension,Shengmai Injection combine with isosorbide mononitrate,bisoprolol,and Danhong Injection could be used; for those with cerebral hemorrhage,Shengmai Injection combined with mannitol and Xingnaojing Injection could be used.Conclusion:The optimal protocols found in this study were basically consistent with the clinical guidelines.However,due to the deficiencies in the data collection attributed to the real-world research and complex actual clinical situation,the clinical medication protocols provided in this study could be used only for reference,and many clinical studies are still needed to explore more effective combined medication protocols.
