To systematically evaluate the efficacy and safety of Bufei Huoxue Capsules combined with western medicine in the treatment of chronic obstructive pulmonary disease(COPD).Methods:Computers were searched for Embase，PubMed，Cochrane Library，SinoMed，CNKI，VIP，and WANFANG DATA databases.Randomized controlled trials of Bufei Huoxue Capsules for the treatment of COPD were collected.The time limit for the search was from building database to December 2017.RevMan.5.3 software was used for data analysis，and the quality of evidence was evaluated according to the international evidence evaluation and recommended GRADE criteria.Results:A total of 7 randomized controlled trials were included with 662 patients with COPD.Meta-analysis showed that the combination of Bufei Huoxue capsules and western medicine in the treatment of COPD was associated with a reduction in the number of acute exacerbations，an increase in 6-minute walking distance(6MWT)，a reduction in cough，sputum，wheezing symptom scores，and an increase in lungs compared with western medicine alone.There was a statistically significant difference in function and SGRQ scores(P<0.05); however，there was no significant difference between the 2 groups in improving total effective rate(P>0.05).After sensitivity analysis，the results were analyzed.A reversal occurred，indicating that the effect had high sensitivity and poor stability.Regarding adverse reactions，no adverse reactions reported by Bufei Huoxue Capsule were reported.Conclusion:Based on the existing clinical evidence，the therapeutic effects of Bufei Huoxue Capsules combined with conventional western medicine treatment of COPD is better than that of western medicine alone.However，due to the low quality of the included studies and the lack of support for large sample evidence，clinical use of this evidence for decision-making still requires caution.
