Due to the high prevalence and disability rate of knee osteoarthritis,as well as the high costs and surgical risks associated with late-stage treatment,early prevention and management of knee osteoarthritis are particularly important.Professor WEN Jianmin believes that the primary pathogenesis of knee osteoarthritis involves the obstruction of qi and blood,the local accumulation of wind,cold,and dampness,and the failure of liver and kidney meridian qi to properly flow.The treatment should prioritize the restoration of qi and blood,with a focus on the liver and kidneys,gently removing obstructions,softening the muscles to promote movement,warming the meridians to dredge collaterals,and realigning the bones to regulate the tendons.The specific treatment approach includes six steps:first,using scraping and kneading techniques around the patella to promote qi and blood circulation,soften the muscles,and restore skeletal mechanical balance.Next,applying techniques such as plucking,scraping,and muscle separation to relax the tendons and dredge collaterals,nourish qi and blood,and improve blood circulation in the lower limbs and local area.Finally,performing calming and organizing techniques to expel pathogenic factors,dredge collaterals,nourish the tendons and vessels,enhance muscle strength,and restore muscle elasticity.These techniques are collectively referred to as the “six-step manipulation”.At the same time,Professor WEN emphasizes the importance of appropriate exercise to enhance the body's self-regulation,as a crucial measure for nourishing the tendons and strengthening the bones in the prevention and treatment of knee osteoarthritis.The promotion and application of the six-step manipulation provides a systematic and standardized operational approach for manual therapy in the early stages of knee osteoarthritis.
