      目的:探讨白介素18和脂联素在2型糖尿病肾病(DN)中的水平变化及意义。方法:选择97例2型糖尿病患者,根据尿白蛋白排泄率(UAER)分为3组:正常蛋白尿组,微量蛋白尿组和大量蛋白尿组,另选健康体检者35例为对照组。采用酶联免疫吸附法(ELISA)检测血清脂联素和白介素18(Interleukin-18,IL-18),比较各组间的差异。结果:3组患者血清IL-18明显高于正常对照组(P<0.05),正常蛋白尿组血清脂联素低于对照组,微量蛋白尿组、大量蛋白尿组血清脂联素高于对照组,差异有统计学意义(P<0.01)。IL-18与病程、SBP、HbA1c、FPG、TG、TC、LDL-C、BUN、Scr、UAER呈不同程度正相关(P<0.01),与HDL-C呈负相关(P<0.01),与其他指标无相关性。脂联素与病程、SBP、BUN、Scr、UAER呈正相关(P<0.01)。结论:IL-18和脂联素与2型DN的发生、发展有关,二者联合检测有助于DN早期诊断及临床治疗监测。
English Summary:
      To investigate the changes and significance of interleukin-18(IL-18) and adiponectin in type 2 diabetic patients with nephropathy. Methods:This study included 96 patients with type 2 diabetes mellitus,These patients were divided into three groups according to the urinary albumin excretion rate(UAER) : normalalbuminuria group,micro-albuminuria group and macro-albuminuria ,and 35 healthy subjects as control group .Serum adiponectin and IL-18 was measured by enzyme linked immunosorbent assay(ELISA), Compare the differences of the groups. Results:Serum IL-18 levels in the diabetic patients were significantly higher than those in controls (P<0.05). Serum adiponectin levels in the normalalbuminuria group lower than in control's, micro-albuminuria group and macro-albuminuria higher than in control's(P<0.01),and increased with UAER.Serum IL-18 concentrations were significantly positively correlated with duration of DM. SBP、HbA1c、FPG、TG、TC、LDL-C、BUN、Scr and UAER levels (P<0.01) and significantly inversely correlated with HDL-C, were not obviously correlated with other indicator. adiponectin concentrations were significantly positively correlated with duration of DM. SBP、BUN、Scr and UAER levels (P<0.01). Conclusion:IL-18 and adiponectin may be involved in the development of type 2 diabetic nephropathy, combined detection of both have important role in early diagnosis and clinical treatment monitoring.
