目的:观察补肝益肾方对高血压病早期肾损害患者尿微量蛋白、血浆一氧化氮(NO)及内皮素-1(ET-1)的影响,探讨其疗效及机制。方法:将164例高血压病早期肾损害患者按数字表法随机分为对照组和治疗组,各82例,2组均给予包括厄贝沙坦在内的降压方案积极控制血压,治疗组联合补肝益肾方治疗,3个疗程后对比2组治疗前后动态血压、尿微量蛋白含量及NO、ET-1血清水平。结果:2组24 h平均收缩压、24 h平均舒张压均较治疗前明显下降,治疗组24 h收缩压变异系数、24 h舒张压变异系数均较治疗前明显下降,同时较对照组下降更为明显,差异均有统计学意义(P<0.05);2组尿微量蛋白含量及ET-1血清水平均较治疗前下降,治疗组均较对照组下降更为明显,2组NO血清水平均较治疗前升高,治疗组升高更为明显,差异均有统计学意义(P<0.05)。结论:补肝益肾方治疗高血压病早期肾损害,可平稳控制血压,减少尿微量蛋白排泄,抑制ET-1释放,提高NO合成,改善血管活性,抑制病情进展。 |

To observe the effect of Bugan Yishen (nourishing liver and kidney) formula on urinary protein,plasma nitric oxide (NO) and endothelin -1 (ET-1) of patients with early hypertension renal damage and explore its effect and mechanism. Methods: One hundred and sixty four patients with early renal damage of hypertension were randomly divided into control group (n=82) and observation group (n=82). Both groups gave antihypertensive therapy including Maher Bbe Chatain to positively control blood pressure,and patients in the treatment group were also Bugan Yishen formula. After 3 courses of treatment,the dynamic blood pressure,urine trace protein content and NO,ET-1 serum levels of the two groups before and after treatment were compared. Results: The 24h average systolic pressure,diastolic pressure of 24 h of patients in both groups were significantly decreased after treatment; 24h systolic pressure,diastolic pressure of 24h coefficient of variation of the treatment group were significantly decreased after treatment,and decreased more significantly than the control group,and the differences were statistically significant (P<0.05); the urinary protein content and ET-1 of patients in both groups decreased after treatment,and the treatment group decreased more significantly; serum NO levels of groups increased after treatment,and the treatment group increased more significantly and the differences were statistically significant (P<0.05). Conclusion: Bugan yishen formula treating renal lesion in early stage of hypertension can stably control blood pressure,reduce urinary protein excretion,inhibit the release of ET-1,increase NO synthesis,improve vasoactive,and inhibit the progress of the disease. |
