目的:观察蠲痹方外用治疗类风湿性关节炎(Rheumatoid Arthritis,RA)的疗效及患者生活质量的影响。方法:选取2013年10月至2015年6月唐山工人医院收治的86例RA活动期患者,随机分为对照组和观察组,每组43例,对照组予美洛昔康和甲氨蝶呤,观察组加用蠲痹方外敷关节及穴位,30 d为1个疗程。比较2组临床指标、实验室指标,采用美国风湿病学会制定的ACR20、ACR50、ACR70来评价疗效,SF-36量表评价生活质量,记录不良反应。结果:与对照组比较,治疗后观察组晨僵时间、关节疼痛数、20 m步行时间,疼痛VAS评分、医生VAS评分、患者VAS评分、HAQ评分及CRP更低,SF-36量表中躯体疼痛、总体健康及活力改善更为明显(P<005);ACR20、ACR50、ACR70分别为814%、651%和359%,高于对照组的605%、395%和140%(P<005);2组不良反应发生率相近。结论:蠲痹方外用治疗RA安全有效,值得临床推广。

English Summary:

To Observe of Juanbi square with the treatment of rheumatoid arthritis (Rheumatoid Arthritis, RA) influence the efficacy and quality of life of patients.Methods:A total of 86 cases of RA patients were selected from October 2013 to June 2015 in Tangshan workers' hospital, were randomly divided into control group and observation group, 43 cases in each group, the control group was treated with methotrexate and meloxicam, observation group with Juanbi recipe joint and acupoint, 30 d for 1 courses.The clinical indexes and lab indexes were compared.The curative effect was evaluated by ACR20,ACR50,ACR70 which was made by the United States Institute of Rheumatology.SF-36 scale was used to evaluate the quality of life.The adverse reactions were recorded.Results:Compared with control group, after treatment,the morning stiffness time,the number of joint tenderness,the walking time for 20 miles, VAS score of pain,VAS score of doctor,VAS score of patients, HAQ score and the level of CRP decreased greatly;the bodily pain, general health and vitality improved greatly(P<005);the ACR20,ACR50 and ACR70 was 814%,651% and 359%,respectively;all were higher than that in control group of 605%,395% and 140%(P<005),The incidence of adverse reaction rate was similar between the two groups(P>005).Conclusion:External Juanbi recipe is effective and safe for RA,and is worthy of widely clinical use.
