      目的:探讨养心健脑方对下肢矫形手术老年患者术后认知功能障碍的预防作用及对相关生物标记物的影响。方法:选择行下肢矫形手术的ASAⅠ~Ⅱ级的老年患者50例,随机分为对照组和观察组,行下肢矫形术后口服养心健脑方免煎颗粒,术后1 d、手术后第3天及第7天采用简易精神状态检查表(MMSE)评估认知功能;采用ELISA试剂盒检测蛛网膜下腔穿刺置管成功后(T0)、插入喉罩后即刻(T1)、手术后6 h(T2)、24 h(T3)和48 h(T4)不同时间点(Aβ)1-42的结果。统计分析评价2组患者的MMES术前术后评分改变以及(Aβ)1-42标记物浓度的差异。MMSE下降2分以上为认知功能下降,Aβ(1-42)蛋白水平的升高则认为应激反应增加。结果:认知评分结果显示,MMSE量表的得分,观察组均高于对照组,2组分别与术前比较,差异有统计学意义(P<0.05);ELISA检测结果显示,手术后6 h(T2)、24 h(T3)和48 h(T4)与T0比较,2组患者术后6 h Aβ(1-42)蛋白水平显著升高(P<0.05);术后24 h Aβ(1-42)蛋白水平,观察组明显低于对照组(P<0.05)。结论:养心健脑方能显著降低术后患者的认知功能,降低相关生物标记物的释放,值得临床借鉴。
English Summary:
      To observe the effect of intravenous infusion of propofol anesthesia with seven isoflurane anesthesia on postoperative cognitive dysfunction elder patients after lower limb orthopedic surgery.Methods:A total of 60 elder patients with ASA I ~ II orthopedic operation were divided into two groups (blank control group and observation group),and the observation group was administered Yangxin Jiannao granules.After 1 day,3 day and 7 days′s operation,using mini mental state examination (MMSE) to assess cognitive function.Use ELISA kit for detection of spinal subarachnoid puncture catheterization success (T0),insert after laryngeal mask immediately (T1),after surgery (T2) 6 h,24 h (T3) and 48 h (T4) at different time points (a beta) 1-42.Results:The cognitive scores showed that in regards to MMSE score,the observation group were higher than those of the control group,and the preoperative comparison differs significantly(P<0.05).ELISA results showed that,after surgery (T2) 6 h,24 h (T3) and 48 h (T4) and t0 comparison,two groups of patients after 6 h of amyloid beta (1-42) protein levels both increased significantly(P<0.05).In regards to postoperative 24 h A-beta (1-42) protein levels,the observation group was significantly lower than that of the control group (P<0.05).Conclusion:Yangxin Jiannao decoction can significantly reduce postoperative cognitive function of the patients,reduce the release of biological markers,and can be used for clinical application.
