      目的:将平衡针法与虚拟现实技术结合治疗脑卒中偏瘫患者,并进行观察和分析其临床疗效。方法:随机选取2015年12月1日至2016年12月1日在辽宁中医药大学附属第二医院就诊的脑卒中伴偏瘫患者86例,其中男49例,女37例;年龄30~75岁,平均年龄(52.34±12.34)岁。根据患者就诊先后顺序分别纳入观察组与对照组,对照组虚拟现实技术功能训练,观察组在对照组机场上结合平衡针,2组患者均在干预治疗的前后进行Fugl-Meyer量表评定患者肢体功能,改良Barthel指数评定量表(MBI)来评价患者的日常生活能力,Prokin平衡训练仪评定患者平衡功能,根据其结果对其疗效进行评价,同时运用酶联免疫吸附(ELISA)检测2组患者治疗前后血清脑源性神经营养因子(BDNF)的浓度变化。结果:在进行了为期4周的治疗后,观察组的总有效率为93.02%,对照组的有效率为74.42%,观察组患者的肢体运动功能、日常生活能力、平衡能力较治疗前有显著的提高(P<0.05),且与对照组比较,差异有统计学意义(P<0.05)。且其血清BDNF经过4周治疗后2组患者数值均较前提高(P<0.05),且观察组明显优于对照组(P<0.05)。结论:平衡针法结合虚拟现实技术治疗可以改善脑卒中偏瘫患者的运动及平衡功能,与传统的针刺加常规康复治疗更有利于患者的恢复。
English Summary:
      To observe and analyze the clinical effect of balance acupuncture with virtual reality technique in the treatment of hemiplegia after stroke. Methods: A total number of 86 patients with hemiplegia after stroke were randomly selected from December 1, 2015 to December 1, 2016, in Department of Rehabilitation and Neurology of the hospital. 37 cases were female and 49 were male, ranging from 30 to 75 years old, with the average age of (52.34±12.34). According to the visit order, patients were randomly divided into the observation group and control group. The virtual reality technique was used in the control group, and the observation group was additionally treated with balance acupuncture. Limb function of patients in the two groups before and after intervention were assessed by Fugl-Meyer scale, modified Barthel index (MBI) was used to evaluate daily living activity and Prokin balance training device was applied to evaluate balance function. The curative effects were evaluated according to the above results and the changes in serum brain-derived neurotrophic factor (BDNF) concentration of patients in the two groups were tested by enzyme linked immunosorbent assay (ELISA) at the same time. Results: After 4-week treatment, the total effective rate of the observation group was 93.02%, while that of the control group was 74.42%. Limb motor function, daily living activity, balance function of patients in the observation group after the treatment were significantly improved (P<0.05), which showed significant difference compared with the control group (P<0.05). The serum BDNF after 4-week treatment of both the two groups were higher (P<0.05), and the observation group was superior to the control group (P<0.05). Conclusion: The treatment of balance acupuncture combined with virtual reality technique may improve the movement and balance function for stroke patients, and receive better effects than the traditional acupuncture combined with conventional rehabilitation treatment.
