| 目的:研究一指禅推拿、弹拨理筋手法配合针刀治疗膝关节骨性关节炎临床疗效。方法:选取2015年4月至2016年3月在北京中医医院推就诊的膝关节骨性关节炎患者60例,将其随机分为2组,其中观察组30例,采用一指禅推拿、弹拨理筋手法配合针刀治疗,每周做1次针刀,每周做2次手法,15 min/次,连续治疗5周为1个疗程;对照组30例,每2周关节腔注射1次医用几丁糖,每周做2次手法,15 min/次,5周为1个疗程。所有患者于6周后进行疗效评定。结果:统计学分析结果显示,观察组患者整体有效率为93.33%,对照组患者整体有效率为80.0%,有效率远高于对照组(P<0.05);经过2周、4周的治疗后,观察组患者的疼痛、僵硬和关节功能的症状评分要明显低于对照组患者(P<0.05),观察组患者恢复情况优于对照组患者;组患者的生理功能、生理职能、健康状况、精力得分明显高于对照组患者,躯体疼痛评分明显低于对照组患者(P<0.05),观察组患者的生命质量优于对照组患者(P<0.05)。结论:推拿手法配合针刀治疗膝关节骨性关节炎不仅能够显著改善膝关节骨性关节炎的症状体征,还能够明显提高膝骨关节炎患者的生命质量,是一种高效治疗方法,具有非常重要的临床推广应用价值。 |

| To study clinical curative effect of single thumb massage, plucking reinforcement technique combined with acupotomy in the treatment of knee osteoarthritis. Methods:A total of 60 cases of knee joint osseous arthritis patients who came to Beijing Hospital of TCM from April 2015 to March 2016 were randomly divided into two groups, of which the observation group was of 30 cases, using single thumb massage, plucked reinforcement technique twice a week combined with acupotomy once a week, to do a knife a week for 2 times, each time 15 minutes, for five weeks as a course of treatment; and the control group of 30 cases and applied with intra-articular injection of Carboxymethylchitin every 2 weeks and massage twice a week, each time 15 minutes, for five weeks as a course of treatment. All patients had curative effect evaluation after 6 weeks of treatment. Results:The statistical analysis results showed that in the observation group, patients’ overall efficiency rate was 93.3%, and in the control group 80.0% (P<0.05). After 2 weeks, 4 weeks of treatment, patients of the observation group had lower score in symptoms of pain, stiffness, and joint function, which is better than those of the control group (P<0.05). The observation group’s patients recovered better than the control group with higher score of physiological function, physiological function, health status and energy and lower score of body pain (P<0.05). The patients’ quality of life in the observation group were better (P<0.05). Conclusion:Single thumb massage, plucking reinforcement technique combined with acupotomy can significantly improve not only the symptoms and signs of osteoarthritis of the knee but also the quality of life of patients with knee osteoarthritis, which is an effective treatment method with important clinical value. |
