      目的:运用针刺结合康复训练治疗运动性膝关节损伤,观察其疗效并探讨分析其对膝关节周围肌肉功能及其生物力学的影响。方法:选取2016年1月至2017年1月新疆维吾尔自治区人民医院康复门诊、住院部收治的运动性膝关节损伤患者80例,通过随机数字表法将其分别纳入观察组与对照组,观察组采用针刺结合康复训练,对照组仅予康复训练,2组均在4周治疗后,在等速向心肌力测试下,测定其在角速度分别为60°/s及180°/s时,患侧膝关节的峰力矩、做功量及股后肌群的向心峰值力矩与四头肌的向心峰值力矩的比值(H/Q),同时测定角速度分别为60°/s时,相关肌群的表面肌电情况,并进行Lysholm膝关节功能评定。结果:1)在4周的治疗后,观察组Lysholm膝关节功能评分较前明显提高,差异有统计学意义(P<0.05),且观察组明显优于对照组,差异有统计学意义(P<0.05);2)在角速度为60°/s及180°/s测试时2组峰力矩、作功量及H/Q值均较治疗前明显提高(P<0.05),且观察组明显优于对照组,差异有统计学意义(P<0.05),在角速度60°/S时,观察组的屈伸肌群的sEMG改善程度要明显优于对照组(P<0.05);3)治疗后2组伸肌及屈肌肌力均较前提高,差异有统计学意义(P<0.05),且观察组明显优于对照组,差异有统计学意义(P<0.05)。结论:针刺结合康复训练可以有效的改善运动性膝关节损伤患者的症状,改善膝关节的功能,其机制主要是通过提高膝关节周围肌肉肌力水平和改善其参与膝关节活动的生物力学。
English Summary:
      To observe the effect of acupuncture combined with rehabilitation training in the treatment of exercise induced knee osteoarthritis, and to investigate the effect of the acupuncture on the function and biomechanics of knee joint.Methods:Randomly selected 80 patients with exercise induced knee osteoarthritis in our rehabilitation clinic and inpatient department from January 2016 to January 2017.Then were then divided into observation group and control group by random number table method.The observation group was treated with acupuncture combined with rehabilitation training, and the control group only received rehabilitation training.After treatment of 4 weeks, conduct isokinetic muscle strength test in determination of the angular velocity were 60°/s and 180°/s of the two groups respectively.Also, test the ratio of muscle peak torque and four muscles centripetal centripetal peak torque and peak torque, work load of femoral knee joints after (H/Q), simultaneous determination of angular velocity at 60°/s, besides surface EMG related muscles, and knee joint function evaluation Lysholm.Results:1) After 4 weeks of treatment, the observation group Lysholm score of knee joint function was obviously improved (P<0.05), and the observation group was significantly better than the control group (P<0.05); 2) Angular velocity of 60°/s and 180°/s when tested two groups of peak torque, work load and the H/Q values were significantly increased (P<0.05), and the observation group was significantly better than the control group (P<0.05), when the angular velocity is 60°/s, the observation group of flexor and extensor sEMG improvement was significantly better than the control group (P<0.05); 3) the two groups after treatment of extensor and flexor muscle strength were improved (P<0.05), and the observation group was significantly better than the control group (P<0.05).Conclusion:Acupuncture combined with rehabilitation training can effectively improve the symptoms of patients with knee injury and the function of knee joint.The mechanism is mainly by increasing the knee muscle strength and improving the level of their participation in the activities of the biomechanics of knee joint.
