目的:分析黄连降糖汤治疗2型糖尿病的临床疗效及对血管VEGF的影响。方法:选取2015年1月至2016年12月张家港市中医医院确诊治疗的2型糖尿病患者80例,按照随机数字表法随机分为对照组和观察组,每组40例。对照组服用盐酸二甲双胍肠溶胶囊治疗,观察组服用黄连降糖汤治疗,2组均治疗2个月。观察2组患者的临床疗效、血糖血脂指标包括空腹血糖(FPG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白(HbA1C)、血脂总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)水平。检测治疗前后VEGF水平变化情况。结果：观察组总有效率为90.00%;对照组总有效率为72.50%。经过治疗后,患者的FPG、2 h PBG、HbA1c、TC、TG、LDL-C水平均呈现降低趋势,HDL-C水平则明显增加。观察组的血糖血脂水平明显优于对照组(P<0.05)。观察组治疗后VEGF水平为(118.52±21.20)ng/L,对照组为(140.26±27.67)ng/L;观察组明显低于对照组(P<0.05)。结论：黄连降糖汤治疗2型糖尿病安全有效,能够降低血管VEGF水平来减小对内皮细胞的损伤,标本兼顾,值得临床推广。

English Summary:

To analyze the clinical effect of Huanglian Jiangtang Decoction on the treatment of type 2 diabetes mellitus and its effect on the vascular VEGF. Methods:A total of 80 patients who were diagnosed as type 2 diabetes mellitus patients from January 2015 to December 2016 in the hospital were included and randomly divided into two groups. Patients in the control group took metformin hydrochloride enteric-coated capsules, while patients in the observation group received Huanglian Jiangtang Decoction. Patients in the two groups were treated for 2 successive months. The clinical efficacy and blood glucose and blood lipid indexes, including fasting blood glucose (FPG), 2 hour postprandial blood glucose (2 hPBG), glycosylated hemoglobin (HbA1C), total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL-C), high density lipoprotein (HDL-C) level were observed. Changes in VEGF levels were observed before and after the treatment in the two groups. Results:The total effective rate of observation group was 90%, and the total effective rate of control group was 72.50%. After treatment, the patients' FPG, 2 hPBG, HbA1c, TC, TG, LDL-C levels were all showed a decreasing trend, and the HDL-C level was significantly increased. The blood glucose and blood lipid levels in the observation group were significantly better than those in the control group (P<0.05). After treatment, the VEGF level of the observation group was (118.52±21.20) ng/L, the control group was (140.26±27.67) ng/L; the observation group was significantly lower than the control group (P<0.05). Conclusion:Huanglian Jiangtang Decoction is safe and effective in the treatment of type 2 diabetes. It can reduce the vascular VEGF level to prevent damages to endothelial cells in certain degree, which is worthy of clinical application.
