| 目的：探讨针灸联合中药治疗治疗腰椎间盘突出症临床疗效及其对患者血清IL-1β、MMP-3及TNF-α水平的影响。方法:选取2015年3月至2016年6月河南省省直第一医院收治的腰椎间盘突出症患者103例,随机分为对照组(n=51)和观察组(n=52),对照组给予自拟中药通络益肾方治疗,观察组在对照组的基础上联合针灸治疗,2组均连续治疗4周。统计2组临床疗效;比较治疗前后2组简式疼痛量表(MPQ)评分、汉化Osweatry功能障碍指数(ODI)及腰椎功能评分;检测比较治疗前后2组血清炎性因子水平。结果:观察组临床总有效率为92.31%,较对照组的76.47%显著提高(P<0.05);与治疗前比较,治疗后2组视觉模拟评分量表(VAS)评分、感觉疼痛分级指数(PRI)评分、感情PRI评分、ODI及血清白细胞介素-1β(IL-1β)、金属基质蛋白酶-3(MMP-3)、肿瘤坏死因子-α(TNF-α)水平均明显降低,且观察组显著低于对照组(P<0.01);而2组腰椎功能评分均明显升高,且观察组显著高于对照组(P<0.01)。结论:针灸联合中药可显著降低腰椎间盘突出症患者血清IL-1β、MMP-3及TNF-α水平,抑制机体炎性反应及椎间盘退变,明显减轻机体疼痛,促进腰椎功能恢复,疗效显著优于单纯中药治疗。 |

| To explore the clinical efficacy of acupuncture combined with Chinese herbal medicine on the levels of serum IL-1β，MMP-3 and TNF-α in the treatment of lumbar disc herniation.Methods:A total of 103 cases with lumbar disc herniation who were admitted and treated in our hospital from March 2015 to June 2016 were selected and divided into control group (n=52) and observation group (n=52).The patients of control group were treated with Tongluo Yishen formula，and the patients of observation group were treated with acupuncture based on the conventional treatment.The 2 groups were treated continuously for 4 weeks.The efficacy of 2 groups were recorded and the MPQ scores，ODI and lumbar function scores were compared; the inflammatory factors levels were determined before and after treatment of 2 groups.Results:The clinical total effective rate of the patients in the observation group was 92.31%，which was higher than that 76.47% of control group (P<0.05); Compared with before treatment，the VAS scores，feeling and emotion PRI scores，ODI and the levels of serum IL-1β，MMP-3 and TNF-α decreased significantly，and those in observation group were lower than that in control group (P<0.01); besides that，the lumbar function scores increased significantly in 2 groups，and that in observation group was higher than that in control group (P<0.01).Conclusion:Acupuncture combined with Chinese herbal medicine can reduce the levels of serum IL-1β，MMP-3 and TNF-α，and restrain the inflammation and intervertebral disc degeneration，and reduce the pain and promote the lumbar functional recovery.The efficacy is significantly better than Chinese herbal medicine treatment alone. |
