目的:探讨当归子饮对寻常型银屑病患者皮肤屏障功能及血流变的影响。方法:选取2015年8月至2017年7月广东省中西医结合医院收治的寻常型银屑病患者114例为研究对象,随机分为观察组和对照组,每组57例。2组均给予常规对症治疗,观察组在此基础上给予当归子饮治疗,4周为1个疗程,2组均连续治疗2个疗程。比较2组患者临床疗效,治疗前后皮肤屏障功能、血清炎性反应因子及血流变学指标。结果:治疗后观察组与对照组的总有效率分别为94.74%、80.70%,观察组显著高于对照组(P<0.05);与治疗前比较,治疗后2组皮脂含量、角质层含水量均显著升高,且观察组显著高于对照组(P<0.01);2组经表皮水分丢失量显著降低,且观察组显著低于对照组(P<0.01);与治疗前比较,治疗后2组血清IL-2、IL-8、IFN-γ水平均显著下降(P<0.01),且观察组显著低于对照组(P<0.05或P<0.01);2组血清IL-4水平均显著升高,且观察组显著高于对照组(P<0.01);与治疗前比较,治疗后2组高/中/低切全血黏度、血浆黏度、红细胞比容等均显著下降(P<0.05或P<0.01),且观察组显著低于对照组(P<0.01)。结论:当归子饮可有效改善寻常型银屑病患者皮肤屏障功能,降低血清炎性反应因子水平,促进血流变学恢复正常,临床疗效优于单用常规西药。

English Summary:

To explore the effect of danggui ziyin on skin barrier function and hemorheology of patients with psoriasis vulgaris.Methods:A total of 114 cases of patients with psoriasis vulgaris in our hospital were selected as the research objects,and were randomly divided into the observation group and the control group,with each group for 57 cases.2 groups were given routine symptomatic treatment.The observation group was treated with the danggui ziyin on the basis of that,4 weeks for 1 course,and 2 groups were treated continuously for 2 courses.The clinical efficacy of 2 groups,the skin barrier function,serum inflammatory factors and hemorheological indexes before and after treatment were compared.Results:The total effective rates of the observation group and the control group were respectively 94.74%,80.70%.The observation group was significantly higher than the control group (P<0.05).Compared with those before treatment,the sebum content and cuticle moisture content of 2 groups significantly increased after treatment,and the observation group was significantly higher than the control group (P<0.01).The water loss through the epidermis significantly reduced,and the observation group was significantly lower than the control group (P<0.01).Compared with those before treatment,the serum IL-2,IL-8,IFN-γ levles of 2 groups after treatment significantly decreased,and the observation group was significantly lower than the control group (P<0.05 or P<0.01).The serum IL-4 level of 2 groups significantly increased,and the observation group was significantly higher than the control group (P<0.01).Compared those with before treatment,the high/medium/low shear whole blood viscosity,plasma viscosity and hematocrit of 2 groups after treatment significantly decreased (P<0.05 or P<0.01),and the observation group was significantly lower than the control group (P<0.01).Conclusion:Danggui ziyin can effectively improve skin barrier function of patients with psoriasis vulgaris,reduce the serum inflammatory factors levels,promote the hemorheology return to normal.And the clinical effect is better than conventional western medicine alone.
