      目的:探讨益气健胃化痰方联合兰索拉唑肠溶胶囊治疗慢性胃炎患者的疗效及对免疫功能和炎性反应递质的影响。方法:选取2016年6月至2017年6月榆林市中医医院收治的慢性胃炎患者106例,按照随机数字表法随机分为对照组和观察组,每组53例。对照组采用兰索拉唑肠溶胶囊治疗,观察组在对照组基础上联合益气健胃化痰方治疗。2组疗程均为4周。比较2组的疗效,治疗前后Hp阳性情况、主要症状积分、免疫功能和炎性反应递质水平变化。结果:观察组总有效率(92.45%)高于对照组(71.69%),差异有统计学意义(P<0.05)。2组治疗后Hp阳性率降低,差异有统计学意义(P<0.05);观察组治疗后Hp阳性率低于对照组,差异有统计学意义(P<0.05)。2组治疗后胃脘胀满、胃脘疼痛、食后饱胀积分降低,差异有统计学意义(P<0.05);观察组治疗后胃脘胀满、胃脘疼痛、食后饱胀积分低于对照组,差异有统计学意义(P<0.05)。2组治疗后CD3+、CD4+和CD4+/CD8+升高,差异有统计学意义(P<0.05);观察组治疗后CD3+、CD4+和CD4+/CD8+高于对照组,差异有统计学意义(P<0.05)。2组治疗后白细胞介素-6(IL-6)、白细胞介素-17(IL-17)和转化生长因子-β1(TGF-β1)下降,差异有统计学意义(P<0.05);观察组治疗后IL-6、IL-17和TGF-β1低于对照组,差异有统计学意义(P<0.05)。结论:益气健胃化痰方联合兰索拉唑肠溶胶囊治疗慢性胃炎患者疗效显著,可增强患者细胞免疫功能及减轻细胞炎性反应。
English Summary:
      To investigate the effects of different concentrations and different administration time of salvianolic acid B (SalB) on regulating TLR4-NFκB-TNFα inflammatory pathway in hypoxia neonatal rat cardiomyocytes.Methods:Cardiomyocytes of Wistar neonatal rats that were born in 1 to 3 days were isolated and purified,and hypoxia model was established in vitro.High,medium,and low doses of SalB concentrations were 10-5 mol/L,10-6 mol/L and 10-7 mol/L,and were administered at the same time of,and 6 hours before and after hypoxia respectively.TLR4 and NFκB mRNA expressions were detected by qRT-PCR; TLR4 and NFκB protein expressions were detected by immunohistochemical method; the concentration of supernatant TNFα was detected by ELISA.Results:Compared with normal group,the expressions of TLR4 and NFκB mRNA and protein,and the concentration of TNFα in supernatant were significantly increased in hypoxia model group (P<0.05 or P<0.01).Compared with the hypoxia model group,the expressions of TLR4 and NFκB mRNA and protein,and the concentration of TNFα in supernatant were significantly decreased in high and medium doses of salvianolic acid B administered before and meantime of hypoxia group (P<0.05 or P<0.01).The most significant decrease was observed in the high-dose group administered before hypoxia (P<0.01).Conclusion:TLR4-NFκB-TNFα inflammatory pathway was significantly activated 6 h after hypoxia injury.Salvianolic acid B administered before and the same time of hypoxia protected cardiomyocytes tremendously.This effect related to the inhibition of TLR4-NFκB-TNFα inflammatory pathway.Salvianolic acid B showed dose-effects and time-effects,and the best effect was observed in the high-dose group administered before hypoxia.
