| 目的：观察自拟中药方剂联合二甲双胍治疗胍治疗肝肾阴虚型早期2型糖尿病肾病患者的临床疗效。方法：选取2015年12月至2017年5月重庆两江新区第二人民医院收治的2型糖尿病合并肾病中医辨证肝肾阴虚型患者146例，采用前瞻性随机临床对照研究方法，按随机数字表法分为观察组（自拟中药方剂联合西药治疗）与对照组（西药治疗），每组73例。治疗3个疗程后比较2组患者治疗前后空腹血糖(FBG)、糖化血红蛋白(HbAlc)尿白蛋白排泄率(UAER)、血肌酐（Scr）、超氧化物歧化酶（SOD）、丙二醛（MDA）表达情况。结果：治疗后，2组患者FBG与HbAlc表达均降低（P<0.05），观察组患者FBG与HbAlc表达均低于对照组（P<0.05）。治疗后，2组患者UAER与Scr表达均降低（P<0.05），观察组患者UAER表达均低于对照组（P<0.05）。治疗后，2组患者MDA表达降低，SOD表达升高，观察组MDA表达低于对照组，SOD表达高于对照组（P<0.05）。结论：在西医降糖方案基础上加服自拟中药汤剂能够降低2型糖尿病患血糖水平，减少尿蛋白的排泄，减轻肾脏病理损害，延缓糖尿病肾病发展。其干预途径可能与改善体内氧化应激失衡状态有关。 |

| To observe the curative effects of self-made traditional Chinese medicine prescription combined with Metformin in the treatment of patients with type 2 diabetic nephropathy of yin deficiency of liver and kidney.Methods:A total of 146 patients with Type 2 diabetes nephropathy with yin deficiency of liver and kidney syndrome admitted to our hospital from December 2015 to May 2017 were randomly divided into observation group(self-made traditional Chinese medicine and western medicine)and control group(Western medicine treatment)according to random number table by the prospective randomized controlled trial,with 73 cases in each group.The levels of fasting blood glucose(FBG),urinary albumin excretion rate(HbAlc),serum creatinine(Scr),superoxide dismutase(SOD),dialdehyde(MDA)expression of 2 groups were compared after 3 courses of treatment.Results:The expression of FBG and HbAlc were decreased in both groups after treatment(P<0.05).The expression levels of FBG and HbAlc in the observation group were lower than that of the control group(P<0.05).The levels of UAER and Scr in both groups were decreased(P<0.05),while those in the observation group were lower than that of the control group after treatment(P<0.05).MDA expression decreased and SOD expression increased in the 2 groups,and the MDA level in the observation group was lower than that of the control group after treatment(P<0.05).Conclusion:Based on Western hypoglycemic regimen,blood glucose level,urinary protein excretion and renal pathological damage can be reduced by adding self-made Chinese herbal decoction.In addition,it can delay the development of diabetic nephropathy in type 2 diabetic patients.Its intervention may be related to the improvement of the imbalance of oxidative stress in vivo. |
