| 目的：探讨鼻内窥镜术后复发性上颌窦霉菌感染应用氟康唑注射液联合中药的治疗效果及预后影响。方法：选取2017年1月至2018年1月于河南省直第三人民医院五官科收治的复发性上颌窦霉菌感染病例240例作为研究对象，随机分为观察组和对照组，每组120例。观察组给予鼻内窥镜术后应用氟康唑注射液联合中药治疗，对照组给予鼻内窥镜术后应用生理盐水注射液治疗。通过比较鼻内镜手术术后治疗性效果评估、鼻腔鼻窦结局测试-20（SNOT20）量表评分、2组患者术后30 d上颌窦内各项指标、患者满意度调查结果等评估氟康唑注射液联合中药综合干预的临床效果。结果：观察组鼻内镜手术术后治疗性效果评估均优于对照组，差异有统计学意义（P<0.05）。治疗前后2组鼻腔鼻窦状况均好转（均P<0.05），治疗后观察组鼻窦SNOT20评分明显降低于对照组，差异有统计学意义（P<0.05）。治疗后，观察组患者术后30 d炎性细胞的浸润情况、间质的水肿、黏膜下层腺体和杯状细胞的增生发生情况明显优于对照组，差异均有统计学意义（均P<0.05）。治疗后观察组满意率明显高于对照组，差异有统计学意义（P<0.05）。结论：对于鼻内窥镜术后复发性上颌窦霉菌感染患者，应用氟康唑注射液联合中药的治疗效果和预后影响好，对患者的功能恢复和提高生命质量有重要的意义。 |

| To investigate the therapeutic effects and prognosis of fluconazole injection combined with traditional Chinese medicine for recurrent maxillary sinus fungal infection after endoscopic sinus surgery.Methods:A total of 240 cases of recurrent maxillary sinus fungal infection admitted to the ENT department of the Third People's Hospital of Henan Province from January 2017 to January 2018 were selected as subjects,and they were randomly divided into an observation group and a control group,with 120 cases in each group.The observation group was treated with fluconazole injection combined with traditional Chinese medicine after endoscopic sinus surgery,and the control group was treated with nasal saline injection after endoscopic sinus surgery.By comparing the therapeutic effects evaluation after endoscopic sinus surgery,sinus nasal outcome test-20 (SNOT20) scale score,the indicators in the maxillary sinus after endoscopic sinus surgery in the 2 groups,the results of patient satisfaction survey,the clinical effects of fluconazole injection combined with traditional Chinese medicine intervention were evaluated.Results:The postoperative therapeutic effects evaluation in the observation group after endoscopic sinus surgery was better than that in the control group,and the difference was statistically significant (P<0.05).The nasal sinus was improved in both groups before and after treatment (P<0.05).After treatment,the sinus SNOT-20 score in the observation group was significantly lower than that in the control group,and the difference was statistically significant (P<0.05).The infiltration of inflammatory cells,interstitial edema,submucosal gland and goblet cell hyperplasia in both groups after treatment were better than those before treatment,and the observation group was significantly better than the control group.The difference was statistically significant (P<0.05).After treatment,the satisfaction rate in the observation group was significantly higher than that in the control group,and the difference was statistically significant (P<0.05).Conclusion:For the patients with recurrent maxillary sinus infection after endoscopic sinus surgery,the fluconazole injection combined with traditional Chinese medicine has a good effect on the treatment effect and prognosis of patients,which is of great significance to the function recovery and improving the quality of life of the patients. |
