| 目的:分析糖尿病肾病Ⅲ期患者在经过益肾活血泄浊方联合陆氏针灸治疗前后,患者外周血中炎性细胞因子的变化。方法:选取2017年1月至2018年12月上海中医药大学附属第七人民医院收治的血糖控制稳定的糖尿病肾病Ⅲ期(CKD1~3期;脾肾气虚、湿浊瘀阻型)患者90例作为研究对象,按照随机对照的原则分为A、B、C 3组，每组30例。A组给予西药常规治疗;B组在西药治疗基础上加益气活血泄浊方口服;C组在中西药治疗的基础上加上针灸治疗。3组都以2个月为1个疗程,共治疗3个疗程,分别跟踪测量3组对象外周血中的血糖FBG、2 hPG以及糖化血红蛋白HbA1c等血糖指标和炎性细胞因子(MCP-1、TGF-β1、TNF-α)。结果:治疗后3个疗程后,3组患者的血糖FBG、2 h PG以及糖化血红蛋白HbA1c等血糖指标均有所改善,炎性细胞因子(MCP-1、TGF-β1、TNF-α)表达水平均有所降低。C组的改善效果较其他2组差异有统计学意义(P<0.05),A组与B、C组比较,差异均有统计学意义(P<0.05)。结论:3组患者在治疗前后炎性细胞因子表达水平均有一定程度降低,C组患者的炎性细胞因子表达水平降低最多,体内炎性反应改善最为明显。 |

| To analyze the changes of inflammatory cytokines in the peripheral blood of patients with phase III diabetic nephropathy before and after the treatment of Yishen Huoxue Xiezhuo decoction combined with Lu's acupuncture and moxibustion.Methods:A total of 90 patients of phase III diabetic nephropathy(CKD1-3 phase; spleen and kidney Qi deficiency and dampness turbid stasis type)admitted to the Seventh People's Hospital of Chinese medicine from January 2017 to December 2018 were selected as subjects.According to the principle of randomized control,they were divided into group A:western medicine control group was given conventional western medicine,group B:Chinese and western medicine treatment group was given Yishen Huoxue Xiezhuo decoction on the basis of western medicine treatment,group C:Chinese and western medicine combined with Lu's acupuncture and moxibustion treatment group on the basis of Chinese and western medicine.All of the 3 groups were treated with 2 months as a course of treatment,a total of 3 courses were treated.The blood glucose FBG,2 hPG,glycosylated hemoglobin HbA1c and other blood glucose indexes and the inflammatory cytokines(MCP-1,TGF-β1,TNF-α)were measured in peripheral blood of 3 groups of subjects.Results:After 3 course of treatment,the blood glucose levels of FBG,2 hPG,glycosylated hemoglobin HbA1c in all 3 groups were improved,and the expression levels of inflammatory cytokines(MCP-1,TGF-β1,TNF-α)were decreased.Compared with the other 2 groups,the improvement effect in group C was statistically significant(P<0.05),and there was significant difference between group A and group B and group C(P<0.05).Conclusion:The expression levels of inflammatory cytokines in the 3 groups are decreased before and after treatment.The expression level of inflammatory cytokines in the Chinese and Western medicine combined with Lu's acupuncture and moxibustion treatment group are the most decreased,and the inflammatory reaction in the body is most obvious. |
