| 目的:探讨化痰行瘀通腑汤保留灌肠联合特布他林治疗慢性阻塞性肺病急性加重期(AECOPD)痰热壅肺型患者的效果观察。方法:选取2014年1月至2016年1月廊坊市中医医院收治的AECOPD患者100例作为研究对象，按照随机数字表法分为对照组与观察组,每组50例。对照组采用特布他林治疗,观察组采用化痰行瘀通腑汤保留灌肠联合特布他林治疗。比较2组患者临床疗效、临床症状和体征、肺通气功能指标、6 min步行试验(6 MWT)、血清生化指标、肠道屏障功能指标、中医症状积分变化及不良反应发生率。结果:观察组的治疗总有效率(920%)显著高于对照组(740%)(P<005)。观察组喘息缓解时间、咳嗽消失时间、喘鸣音消失时间、湿罗音消失时间均显著短于对照组(P<005)。观察组FEV1、FVC、PEF、FEV1/FVC、6MWT均显著高于对照组(P<005)。观察组WBC、N%、IL-6、LTB4均显著低于对照组(P<005)。观察组血清DAO、IFABP、LPS均显著低于对照组(P<005)。观察组中医症状积分变化高于对照组(P<005)。2组患者在治疗过程中均未发生明显不良反应。结论:非化痰行瘀通腑汤保留灌肠联合特布他林可显著改善AECOPD患者临床症状、肺通气功能、肠道屏障功能,且不良反应少。 |

| To investigate the effect of retention enema with Huatan Xingyu Tongfu decoction combined with terbutalineon on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)of phlegm heat obstructing the lung syndrome.Methods:A total of 100 patients with AECOPD admitted to Langfang Hospital of Traditional Chinese Medicine from January 2014 to January 2016 were selected as the research subjects.Patients were randomly divided into an observation group and a control group，with 50 patients in each group.The control group was treated with terbutaline，while the observation group was treated with retention enema of Huatan Xingyu Tongfu decoction combined with terbutaline.The clinical efficacy，clinical symptoms and signs，pulmonary ventilation function index，6-minute walking test，serum biochemical index，intestinal barrier function index，changing rate of TCM symptom score and incidence of adverse reactions were compared between the 2 groups.Results:The total effective rate of the observation group(920%)was significantly higher than that of the control group(740%)(P<005).The relieving time of wheezing，disappearing time of cough，disappearing time of wheezing and disappearing time of moist rales in the observation group were significantly shorter than those in the control group(P<005).The FEV1，FVC，PEF，FEV1/FVC and 6MWT in the observation group were significantly higher than those in the control group(P<005).The WBC，N%，IL-6 and LTB4 in the observation group were significantly lower than those in the control group(P<005).The serum DAO，IFABP and LPS in the observation group were significantly lower than those in the control group(P<005).The changing rate of TCM symptom score in the observation group was higher than those in the control group(P<005).There were no significant adverse reactions in both groups.Conclusion:Huatan Xingyu Tongfu Decoction retention enema combined with terbutaline can significantly improve the clinical symptoms，lung ventilation function，intestinal barrier function in patients with AECOPD，and fewer adverse reactions |
