| 目的：观察复方紫草油联合氯雷他定治疗轻、中度特应性皮炎的临床疗效。方法：选取2018年4月至2019年3月北京中医药大学东方医院及北京中医药大学深圳医院皮肤科门诊收治的轻、中度特应性皮炎患者77例作为研究对象，按随机数字表分为观察组（n＝39）与对照组（n＝38）。观察组给予氯雷他定联合复方紫草油治疗，对照组给予氯雷他定联合吡美莫司治疗，观察并记录患者所属中医证型，皮损面积及局部红斑、丘疹、表皮剥脱、结痂、鳞屑、肥厚苔藓化、瘙痒程度，计算治疗有效率，记录不良反应发生情况。结果：观察组皮损面积、瘙痒、表皮剥脱、肥厚苔藓化改善疗效与吡美莫司联组相当（P>005），但对红斑、脱屑改善程度有限，总体有效率（758%）低于吡美莫司组（933%）。观察组起效较慢，疗效稳定，不良反应发生率和复发率分别为256%、606%；吡美莫司组起效快，不良反应发生率和复发率分别为1579%、667%，心火脾虚型皮损使用吡美莫司更易出现不良反应。结论：复方紫草油联合氯雷他定可以有效改善轻、中度特应性皮炎患者的皮损状况，对心火脾虚型皮损有显著的安全性优势。 |

| To observe the clinical effects of compound oleum lithospermi combined with loratadine in the treatment of mild and moderate atopic dermatitis(AD).Methods:The effect of different therapy was evaluated through a randomized，open，parallel-group trial.The observation group was treated with oleum lithospermi combined with loratadine，and the control group was given pimecrolimus combined with loratadine.The TCM syndrome types，skin lesion area and the level of erythema，papule，skin peeling，leakage or scabby，scales，hypertrophy and moss of patients were observed and recorded，and the effective rate and the incidence of adverse reactions were calculated.The clinical efficacy of drugs was comprehensively evaluated.Results:The therapeutic effect of compound oleum lithospermi combined with loratadine was close to pimecrolimus combined with loratadine on relieving AD skin lesions area，itching，skin peeling and hypertrophy(P>005)，but it had limited effects on improving erythema and skin peeling.The overall efficiency was 758%，which was less than 933% of pimecrolimus group.But compound oleum lithospermi group had slow action with stable efficacy.The incidence of adverse reactions was 256% and recurrence rate was 606%.The pimecrolimus group had quick action，and had a 1579% of incidence of adverse reactions and 667% of recurrence rate.It is easy to have adverse reactions in the application of pimecrolimus for patients with skin damage of heart-fire hyperactivity and spleen deficiency.Conclusion:Compound oleum lithospermi combined with loratadine can reduce skin lesion of mild and moderate level，and has significant security advantage on patients with TCM syndrome types of heart-fire hyperactivity and spleen deficiency. |
