      目的:探析复方黄芩“水膜”湿敷治疗急性湿疹的疗效及其机制。方法:选取2014年9月至2016年9月武汉市中医医院皮肤科收治的急性湿疹患者60例,按照随机数字表法分为对照组和观察组,每组30例,2组均口服氯苯那敏作为基础治疗;对照组为基础治疗加曲咪新乳膏治疗;观察组为基础治疗加复方黄芩“水膜”湿敷。评估急性湿疹皮损严重程度和瘙痒程度,同时采用流式细胞仪检测自然杀伤细胞(NK),酶联免疫吸附试验(ELISA)检测白细胞介素-2(IL-2)、可溶性白细胞介素2受体(sIL-2R)和5-羟色胺(5-HT)的表达情况。结果:治疗后2组皮损积分和皮损面积均降低,差异有统计学意义(P<005),且观察组皮损积分和皮损面积低于对照组,差异有统计学意义(P<005)。治疗后2组瘙痒积分均有明显降低,差异有统计学意义(P<005),且观察组瘙痒积分低于对照组,差异有统计学意义(P<005)。治疗后2组NK、IL-2均有明显上升,差异有统计学意义(P<005),且观察组NK、IL-2高于对照组,差异有统计学意义(P<005)。治疗后2组5-羟色胺和sIL-2R均有明显下调,差异有统计学意义(P<005),且观察组5-羟色胺和sIL-2R均低于对照组,差异有统计学意义(P<005)。结论:复方黄芩“水膜”能有效改善急性湿疹患者临床皮损和瘙痒等症状,其机制可能与改善患者细胞免疫功能和降低致痒因子5-羟色胺有关。
English Summary:
      To observe the curative effects of wet dressing of scutellaria baicalensis “water film” on acute eczema and explore the therapeutic mechanism. Methods:A total of 60 cases of patients, who were admitted to Wuhan Traditional Chinese Medicine Hospital from 2014 September to 2016 September in dermatology department, were randomly divided into control group and observation group, with 30 cases in each group. Both two groups were given chlorpheniramine as basic treatment. The control group was treated on the basis of the observation group with Triamicinolone Acetonide Acetate and Miconazole Nitrate Neomycin Sulfate Cream. The observation group was treated with wet dressing of scutellaria baicalensis “water film” on the basis of the basic treatment. The acute eczema severity of skin lesions and pruritus was evaluated, and flow cytometry was used to detect natural killer cells (NK), enzyme linked immunosorbent assay (ELISA) detection of interleukin-2 (IL-2) and soluble interleukin 2 receptor (sIL-2R) and five hydroxytryptamine (5-HT) expression.Results:After treatment, the lesions and lesion areas of two groups were significantly decreased (P<005), and the skin lesions and lesion areas in the observation group were lower than those in the control group (P<005). After treatment, the itching scores of two groups were significantly lower (P<005), and the itch score of the observation group was lower than that of the control group (P<005). After treatment, NK and IL-2 of the two groups increased significantly (P<005), and the NK and IL-2 in observation group were higher than the control group (P<005). After treatment, 5-HT and sIL-2R of the two groups were significantly lower (P<005), and 5-HT and sIL-2R of the observation group were lower than the control group (P<005). Conclusion:Wet dressing of scutellaria baicalensis “water film” treatment can effectively improve the clinical skin lesions and pruritus of patients with acute eczema, and its mechanism may be related to the improvement of cellular immune function and the reduction of itching factor 5-HT.
