      目的:通过提炼热敏态腧穴的脑电功率谱密度特征指标,为临床建立热敏态腧穴的脑电客观检测技术奠定科学基础。方法:选取2016年5月至2018年12月江西中医药大学附属医院收治的膝关节骨性关节炎患者30例作为研究对象,以犊鼻穴为试验腧穴,以同侧犊鼻穴正上4寸为对照点,采用热敏腧穴灸感法进行检测,根据施灸过程中是否发生热敏现象分为热敏态腧穴组和非热敏态腧穴组。受试者脑电数据采用美国EGI公司生产的128导高密度脑电系统采集,其采集分为3个时段,每个时段记录15 min,共记录45 min:第1段在艾灸前记录15 min;第2段在艾灸15 min后开始记录15 min;第3段在艾灸结束后开始记录15 min。脑电信号的预处理采用Matlab2014a(美国The MathWorks公司)数据分析软件及其EEGLAB子软件,对灸前、灸中、灸后平均脑电功率谱密度分别进行提取,所得脑电数据采用SPSS 190软件进行统计学分析比较。结果:热敏态与非热敏态腧穴功率谱密度比较:平均功率谱密度差异:热敏态腧穴灸中、灸后与灸前比较,全部导联叠加后的脑电平均功率谱密度有明显增加。进一步分析不同频段脑电功率谱密度分布差异,其中theta、alpha1、alpha 2及beta频段均增加,尤其以alpha1、alpha 2频段增加显著,且beta频段的功率谱密度持续增加至灸后阶段。热敏腧穴不同脑区平均功率谱密度结果提示,灸中、灸后与灸前比较,灸中、灸后双侧额区、顶区、枕区平均功率密度明显增加,而其中央区差异无统计学意义。结论:alpha1、beta节律的能量在热敏态腧穴被激活时占整个脑电能量中的比重较大,而非热敏态腧穴脑电信号变化较少,这可能作为腧穴热敏现象的客观显示标签之一。
English Summary:
      To extract the characteristic indexes of EEG power spectrum decsity of heat-sensitized acupoints,so as to lay a scientific foundation for the establishment of a clinically objective EEG detection technology for heat-sensitized acupoints.Methods:A total of 30 subjects with knee osteoarthritis admitted in the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from May 2016 to December 2018 were included.Dubi(ST 35)was selected as the test acupoint,and the point located at 4 cun higher than the same side Dubi(ST 35)was taken as the control point.The moxibustion sensation method of heat-sensitized acupoints was used detection.The patients were divided into heat-sensitized acupoint group(HA group)and non heat-sensitive acupoint group(NHA group),according to whether the heat-sensitive phenomenon occurred during the moxibustion process.Subjects' EEG data were collected by the 128-conductor high-density EEG system produced by EGI Company in the United States.The collection process was divided into 3 periods,and each period was recorded for 15 min,with a total of 45 minutes.The first period was recorded for 15 min before the moxibustion; the second period was recorded for 15 min after the beginning of moxibustion; and the third period was recorded for 15 min after the end of moxibustion.Matlab2014a(The MathWorks company in the United States)data analysis software and EEGLAB sub-software were used for preprocessing the EEG signals.Average EEG power spectral density was extracted before,during and after the moxibustion,and SPSS 190 software was used for statistical analysis and comparison of the obtained EEG data.Results:Power spectral densities of acupoints in heat-sensitized state and non heat-sensitized state were compared.The difference in average power spectral densities:compared with those before,during and after the moxibustion in acupoints of heat-sensitized state,the average power spectral densities of EEG after the superposition of all the leads were significantly increased.Further analysis of the distribution differences of EEG power spectral densities in different bands:theta,alpha1,alpha2 and beta bands were all increased,especially for the alpha1 and alpha2 bands.The power spectral density of the beta band was increased continuously to the stage after the moxibustion.The results of average power spectral densities of different brain areas of heat-sensitized acupoints indicated that the average power densities at bilateral frontal regions,parietal region and occipital region during and after the moxibustion were significantly increased compared with that before the moxibustion,while there was no statistical difference in that at the central region.Conclusion:The energy of alpha1 and beta rhythms accounted for a large proportion of the whole EEG energy when the heat-sensitized acupoints were activated,while the non heat-sensitized acupoints' EEG signals were less changed,which may be one of the objective display labels of the heat-sensitive phenomenon at acupoints.
