目的：探究桃核承气汤联合中药灌肠治疗慢性肾功能衰竭的临床效果。方法：选取2016年6月至2018年6月辽宁中医药大学附属医院肾脏病科收治的慢性肾功能衰竭患者100例作为研究对象，按随机数字表法将其分为对照组和观察组，每组50例。对照组给予桃核承气汤治疗，观察组在此基础上联合中药灌肠治疗。统计2组患者治疗后临床疗效、肾功能指标、营养指标、炎性反应指标。结果：治疗后观察组与对照组的临床总有效率分别为90.00%、74.00%；与治疗前比较，治疗后2组患者GFR水平显著增高，且观察组显著高于对照组（P<0.05）；治疗后2组患者Scr、BUN水平均显著下降，观察组显著低于对照组（P<0.05）；与治疗前比较，治疗后2组患者HGB、ALB水平显著增高（P<0.05），与对照组比较，观察组患者的HGB、ALB虽高于对照组，但并无统计学意义（P>0.05）；与治疗前比较，治疗后2组患者IL-1、IL-6、hs-CRP、TNF-α水平均显著下降，且观察组显著低于对照组（P<0.01）。结论：桃核承气汤联合中药灌肠可有效缓解慢性肾功能衰竭患者临床症状，改善机体营养状况，其机制与降低炎性因子水平、改善肾功能指标有关。

English Summary:

To explore the clinical effect of Taohe Chengqi Decoction combined with Chinese herbal enema in the treatment of chronic renal failure. Methods:A total of 100 patients with chronic renal failure admitted to our hospital from June 2016 to June 2018 were selected and randomly divided into a control group(n＝50)and an observation group(n＝50). The control group was treated with Taohe Chengqi Decoction, while the observation group was treated with traditional Chinese medicine enema on this basis. The clinical efficacy, renal function, nutrition and inflammation indexes of the 2 groups were analyzed. Results:After treatment, the total clinical effective rates of the observation group and the control group were 90.00% and 74.00%, respectively, and the observation group was significantly higher than the control group(P<0.05); compared with before treatment, the levels of GFR in the 2 groups were significantly increased, and the observation group was significantly higher than the control group(P<0.05); after treatment, the levels of Scr and BUN in the 2 groups were significantly decreased, and the observation group was significantly lower than the control group(P<0.05). After treatment, the levels of HGB and ALB in the 2 groups increased significantly(P<0.05). Compared with the control group, the levels of HGB and ALB in the observation group were higher than those in the control group, but there was no statistical difference(P>0.05). Compared with before treatment, the levels of IL-1, IL-6, hs-CRP and TNF-a in the 2 groups decreased significantly after treatment, and the levels in the observation group were significantly lower than those in the control group(P<0.01). Conclusion:Taohe Chengqi Decoction combined with Chinese herbal enema can effectively alleviate the clinical symptoms of patients with chronic renal failure, and its mechanism is directly related to reducing the level of inflammatory factors and improving renal function indicators.
