| 目的：探讨涤痰汤对脑梗死（CI）疗效及血栓弹力图、血管内皮因子水平影响性。方法：选取2016年12月至2019年3月北京中医药大学房山医院收治的CI的脑梗死患者95例作为研究对象，按就诊顺序随机分为对照组（n=47）和观察组（n=48），对照组常规西药治疗，观察组在对照组基础上加用涤痰汤加减，均治疗4周。观察并比较2组治疗前、治疗后的疗效、血栓弹力图、血管内皮因子、炎性因子、氧化应激指标、中医证候积分等变化。结果：观察组患者治疗后肿瘤坏死因子-α（TNF-α）、C反应蛋白（CRP）、丙二醛（MDA）、人内皮素-1（ET-1）、重组蛋白MMP-9、血小板活化因子（PAF）、最大血块强度（MA）、血细胞凝集块形成速率（α角）、中医证候积分、美国国立卫生研究院卒中量表（NIHSS评分）、血管内皮因子AngⅡ、血管内皮因子GMP-140较对照组下降幅度更大（均P<0.05），而超氧化物歧化酶（SOD）、胰岛素样生长因子-1（IGF-1）、凝血反应时间（R值）、血小板凝集块形成时间（K值）、日常生活能力评分（BI评分）、血管内皮因子NO上升幅度及总有效率均较对照组高（均P<0.05）。结论：涤痰汤能抑制CI炎性因子和氧化应激，抑制血栓形成，保护脑神经，从而改善症状和提高生命质量。 |

| To explore the effects of Ditan Decoction on cerebral infarction(CI)and the levels of thromboelastogram and vascular endothelial factor.Methods:A total of 95 patients with CI from December 2016 to March 2019 in Fangshan Hospital Affiliated to Beijing University of Chinese Medicine were selected as the study objects.According to the order of treatment，they were divided into 2 groups:a control group(n=47)，and a study group(n=48).The control group was treated with conventional Western medicine，and the observation group was treated with modified Ditan Decoction for 4 weeks.The efficacy of the 2 groups when they were enrolled and when the treatment was completed，thromboelastogram，vascular endothelial factor，inflammatory factor，oxidative stress index and TCM syndrome score etc.were observed and compared between the 2 groups.Results:After treatment，TNF-α，CRP，MDA，ET-1，MMP-9，PAF，MA，α angle，TCM syndrome scores，NIHSS score，Ang Ⅱ，GMP-140 in the study group decreased more than those in the control group，while SOD，IGF-1，R value，K value，BI score，NO increase and total effective rate were higher than those in the control group(P<0.05)(P<0.05).Conclusion:Ditan Decoction can inhibit CI inflammatory factors and oxidative stress，inhibit thrombosis，protect brain nerves，improve symptoms and improve quality of life. |
