| 目的:观察川芎茶调散加减方联合针灸治疗周围性面神经麻痹(风寒证)的临床效果。方法:选取2018年2月至2019年2月威海市中医院收治的急性周围性面瘫患者108例作为研究对象,按照随机数字表法分为对照组与观察组,每组54例。对照组给予常规西药治疗,观察组给予川芎茶调散加减方口服联合针灸治疗。比较2组患者主要症状的改善时间、肌电图检查相关指标、面部神经功能分级的变化,测定血清中相关因子水平,比较临床效果。结果:观察组、对照组的有效率92.59%、75.93%,以观察组临床效果更好(P<0.05);观察组患者治疗后的闭目、抬眉、鼓颊的改善时间明显短于对照组(P<0.05);观察组患者的肌电图结果RI潜伏期低于对照组,CMAP波幅高于对照组,面部神经功能分级改善更明显(P<0.05);观察组患者血清中TNF-α、IL-17含量明显低于对照组,GDNF含量显著高于对照组(P<0.05)。结论:川芎茶调散加减方联合针灸利于促进患者临床症状的缓解,改善肌电图指标,促进面部神经功能的恢复,推断其起效可能与通过调控血清中TNF-α、GDNF、IL-17等水平以减轻患者神经病变的炎性反应损伤程度有关。 |

| To observe the clinical effect of Acupuncture combined with Chuanxiong Chatiao San for the treatment of peripheral facial nerve palsy (wind-cold syndrome).Methods:A total of 108 patients with acute peripheral facial paralysis admitted to Weihai hospital of traditional Chinese medicine from February 2018 to February 2019 were selected and divided into a control group and an observation group according to the random number table method，with 54 patients in each group.Patients in the control group were given conventional western medicine treatment，while patients in the observation group were given Acupuncture combined with Chuanxiong Chatiao San.The improvement time of main symptoms，related indexes of electromyography examination and facial nerve function grading were compared between the two groups，the levels of related factors in serum were measured，the clinical effects were compared.Results:The effective rates of the observation group and the control group were 92.59% and 75.93% respectively，and the clinical effect of the observation group was better (P<0.05).The improvement time of eye closure，eyebrow lifting and buccal swelling in the treatment group was significantly shorter than that in the control group (P<0.05).In the observation group，the RI latency was lower than that of the control group，the CMAP amplitude was higher than that of the control group，and the grading improvement of facial nerve function was more obvious (P<0.05).The serum levels of TNF-α and IL-17 in the observation group were significantly lower than those in the control group，and the levels of GDNF were significantly higher than those in the control group (P<0.05).Conclusion:Acupuncture combined with Chuanxiong Chatiao San is helpful to promote the relief of clinical symptoms，improve the emg index，and promote the facial nerve function recovery，its effect may be related to reducing the degree of inflammatory response damage in patients with neuropathy by regulating serum TNF-α，GDNF，IL-17 and other levels. |
