| 目的:观察铍针治疗腰椎退行性骨关节病(Kellgren-Lawrence分型3、4级)的早期临床疗效。方法:选取2014年5月至2015年5月中国中医科学院望京医院收治的腰椎退行性骨关节病患者108例作为研究对象，按照随机数字表法随机分为观察组和对照组，每组54例。观察组使用铍针松解1次/周;对照组口服醋氯芬酸肠溶胶囊,1粒/次，2次/d,3周为1个疗程,如果症状在疗程内消失则停止治疗。分别于治疗前、治疗1周后、治疗2周后、随访1个月、随访3个月，采用VAS及ODI评价治疗效果。结果:2组在治疗第1周后VAS评分比较差异无统计学意义,在第2周后及2次随访时,观察组效果优于对照组，差异均有统计学意义（P<0.05）。2组间ODI分数在前2周无统计学意义,在2次随访时观察组ODI分数与对照组之间差异有统计学意义（P<0.05）。结论:使用铍针治疗腰椎退行性骨关节炎(K-L3、4级)符合微创理念,具有定位精准、操作简便、疗效显著、不影响脊柱的稳定性及小关节正常功能优点,治疗效果优于口服醋氯芬酸肠溶胶囊,为治疗腰椎退行性骨关节炎痛提供了新选择。 |

| To observe the early clinical therapeutic effects of stiletto needle in the treatment of lumbar de-generative osteoarthropathy(grade 3 and 4 of Kellgren-Lawrence classification).Methods:A total of 108 patients with lumbar degenerative osteoarthropathy who were admitted to Wangjing Hospital of China Academy of Chi-nese Medical Sciences from May 2014 to May 2015 were selected as the research objects，and randomly divided into an observation group and a control group according to the random number table，with 54 cases in each group.The stiletto needle was used once per week in the observation group.The control group took aceclofenac enteric-coated capsules orally，1 capsule/time，2 times/d，3 weeks as a course of treatment.The treatment discon-tinued if the symptoms disappeared within the course of treatment.The VAS and the ODI evaluation were used to evaluate the treatment effects before treatment，after 1 week of treatment，after 2 weeks of treatment，followed up for 1 month and 3 months after the end of treatment.VAS and ODI were used to evaluate treatment effects.Results:There was no significant difference in VAS score between the 2 groups after 1 week of treatment，and the effects of the observation group were better than that of the control group，and the differences were statistically significant after 2 weeks of treatment and 2 follow-ups(P<0.05).The ODI scores between the 2 groups was not statistically signifi-cant in the first 2 weeks，but there was a statistically significant difference in ODI score between the observation group and the control group at 2 follow-ups(P<0.05).Conclusion:The use of stiletto needles to treat lumbar degenerative osteoarthropathy(grade K-L 3，4)not only conforms to the minimally invasive concept，but also has the advantages of accurate positioning，simple operation，and significant therapeutic effects，with no influence on the stability of the spine and normal function of the facet joints.The therapeutic effects of stiletto needles are better than that of oral aceclofenac enteric-coated capsules，and it provides a new option for the treatment of lumbar degenerative osteoar-thropathy pain. |
