| 目的：研究温针灸膝四穴配合消肿利水方熏洗治疗膝关节创伤性滑膜炎的临床疗效。方法：选取2018年4月至2019年10月海口市中医医院收治的膝关节创伤性滑膜炎患者90例作为研究对象，按照随机数字表法随机分为熏洗组、温针灸组和联合组，每组30例。熏洗组采用消肿利水方熏洗治疗，温针灸组给予温针灸膝四穴治疗，联合组采用以上2种方式联合治疗。观察并比较各组治疗后临床总有效率、WHOQOL-BREF总积分，以及治疗前后健康状况主观感受积分、生理积分、心理积分、关节积液面积和滑膜厚度等临床指标。结果：熏洗组临床总有效率为66.67%，温针灸组总有效率为73.33%，联合组总有效率为93.33%，3组有效率比较，差异有统计学意义（P<0.05）。治疗后，3组患者WHOQOL-BREF评分均较治疗前明显升高，且联合组WHOQOL-BREF总积分及健康状况主观感受、生理、心理积分均高于熏洗组和温针灸组（均P<0.05），3组关节积液面积和滑膜厚度较治疗前明显减少和变薄（P<0.05），且联合组低于熏洗组和温针灸组（P<0.05）。结论：温针灸膝四穴配合消肿利水方熏洗治疗膝关节创伤性滑膜炎疗效显著，具有较高的应用价值。 |

| To study the clinical effects on the treatment of traumatic synovitis of knee joint using warm acupuncture combined with Xiaozhong Lishui Formula. Methods:An total of 90 patients with traumatic synovitis of knee joint, who were treated in Haikou Traditional Chinese Medicine Hospital for April 2018 to October 2019, according to the random number table method, were divided into 3 groups:a fumigation group, a warm acupuncture group and a combined group. The fumigation group was given the Xiaozhong Lishui Formula, the warm acupuncture group was given warm acupuncture on 4 points of knee, and the combined group was given the both therapies. The clinical indexes of each group were observed and compared in 2 groups. Results:The total effective rates were 66.67 %, 73.33 % and 93.33 %, in the fumigation, warm acupuncture and combined groups, respectively, with statistically significant difference among the 3 groups(P<0.05). After treatment, the WHOQOL-BREF scores of the three groups of patients were significantly higher than before treatment, and the total score of WHOQOL-BREF, subjective feeling of health condition, physiology and psychology in the combined group were higher than those of the fumigation group and the warm acupuncture group(P<0.05). The joint effusion area and synovium thickness in the 3 groups were significantly reduced and thinned compared with those before treatment(P<0.05), and the combined group was lower than the fumigating and warm acupuncture groups(P<0.05). Conclusion:Warm acupuncture on 4 points of knee combined with Xiaozhong Lishui Formula has obvious curative effects on traumatic synovitis of the knee joint and has high application value. |
