| 目的:探讨枳壳甘草汤联合腰椎牵引治疗腰椎间盘突出症疗效。方法:选取2016年1月至2018年9月苏州市中西医结合医院收治的腰椎间盘突出症患者180例作为研究对象，按照随机数字表法随机分为对照组（n=92）和观察组（n=88）。对照组卧床、腰背肌锻炼、腰椎牵引干预1个月。观察组在对照组干预的基础上连续口服枳壳甘草汤1个月。评价比较2组疗效，比较2组治疗前后视觉疼痛模拟评分（VAS）、腰椎功能评定表（JOA）评分、血清免疫球蛋白（Ig）G、IgM、白细胞介素-6（IL-6)、肿瘤坏死因子α（TNF-α）水平。分析观察组患者免疫炎性指标与疼痛、腰椎功能及疗效的关系。结果:与同组治疗前比较，2组治疗后VAS评分和血清IL-6、TNF-α、IgG、IgM水平均降低，同期JOA评分则升高（P<0.05）。与对照组比较，观察组治疗有效率和治疗后JOA评分提高，治疗后VAS评分和血清IL-6、TNF-α、IgG、IgM水平均降低（P<0.05）。观察组血清IL-6、TNF-α、IgG、IgM水平与其VAS评分呈正相关（P<0.05），与其JOA评分和治疗有效率呈负相关（P<0.05）。结论:枳壳甘草汤联合腰椎牵引可安全有效改善腰椎间盘突出症疗效，且其免疫炎性反应与患者疗效相关。 |

| To explore the curative effects of Zhiqiao Gancao Decoction combined with lumbar traction in patients with lumbar disc herniation.Methods:A total of 180 patients with lumbar disc herniation admitted to Suzhou Integrated Traditional Chinese and Western Medicine Hospital were randomly divided into a control group(n=88)and a an observation group(n=92).The control group was treated with bed rest，back muscle exercise，lumbar traction for 1 months.The observation group was treated with Zhiqiao Gancao Decoction for 1 months based on the control group.The curative effects of the 2 groups were evaluated and compared.Before and after treatment，Visual analogue pain score(VAS)，lumbar function assessment scale(JOA)score，serum immunoglobulin(Ig)G，Ig M，interleukin-6(IL-6)and tumor necrosis factor-alpha(TNF-α)levels of the 2 groups wer compared.The relationship between immune inflammation indexes with the pain，lumbar function and curative effects of the observation group were analyzed.Results:Compared with those of the control group，after treatment VAS score and serum levels of IL-6，TNF-α，IgG and IgM of the observation group were decreased(P<0.05).Compared with those of before treatment，the VAS score and serum levels ofIL-6，TNF-α，IgG and Ig M in 2 groups decreased，while in the same period JOA score increased(P<0.05).Serum levels of IL-6，TNF-α，IgG and Ig M were positively correlated with VAS score(P<0.05)，while negatively correlated with JOA score and treatment efficiency(P<0.05).Conclusion:Zhiqiao Gancao Decoction combined with lumbar traction can safely and effectively improve the curative effects of lumbar intervertebral disc herniation，and its immune inflammation is related to the curative effects. |
