      目的:探讨茵栀黄口服液联合布拉酵母菌治疗新生儿高胆红素血症对胆红素水平及神经系统的影响。方法:选取2018年1月至2019年1月皖北煤电集团总医院收治的高胆红素血症新生儿60例作为研究对象,按照随机数字表法分为对照组和观察组,每组30例。对照组患儿予以蓝光照射治疗,观察组在对照组基础上予以茵栀黄口服液(1/3支/次,3次/d)联合布拉酵母菌(0.25 g/次,2次/d)口服治疗,2组均连续治疗6 d。比较2组治疗后的临床疗效;检测并比较治疗前与治疗36、72、108与144 h后2组经皮胆红素(TcB)水平;比较2组治疗前后神经系统异常情况,包括脑干听觉诱发电位(BAEP)、振幅整合脑电图(aEEG)与头颅磁共振成像(MRI);比较2组不良反应发生率。结果:观察组临床有效率(93.33%)显著高于对照组(66.67%),差异有统计学意义(P<0.05);与治疗前比较,治疗后36、72、108与144 h,2组TcB水平均显著降低,随时间推移,呈逐渐降低趋势,且观察组低于对照组,差异有统计学意义(P<0.05);与治疗前比较,治疗后2组BAEP、aEEG与MRI检查异常率均显著降低,且观察组显著低于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率(30.00%)与对照组(16.67%)比较,差异无统计学意义(P>0.05)。结论:茵栀黄口服液联合布拉酵母菌治疗新生儿高胆红素血症疗效显著,可显著降低TcB水平,同时改善患儿的神经系统损害,有利于预后,且安全性好。
English Summary:
      To investigate the effects of Yinzhihuang Oral Liquid combined with Saccharomyces boulardii on bilirubin level and nervous system in neonates with hyperbilirubinemia.Methods:A total of 60 neonates with hyperbilirubinemia admitted to Wanbei Coal and Electricity Group General Hospital from January 2018 to January 2019 were selected and divided into a control group(n=30)and an observation group(n=30)according to random number table method.The child patients in the control group were treated with blue light irradiation,and those in the observation group was given Yinzhihuang Oral Solution(1/3 each time,3 times/d)combined with Saccharomyces boulardii(0.25 g/time,2 times/d)oral treatment.Both groups were treated continuously for 6 d.The clinical efficacy of the 2 groups were compared after treatment; the levels of transcutaneous bilirubin(TcB)before and after 36,72,108 and 144 h of treatment were detected and compared; the neurological abnormalities before and after treatment in 2 groups [including brainstem auditory evoked potential(BAEP),amplitude-integrated electroencephalogram(aEEG)and magnetic resonance imaging(MRI)] were compared; the incidence of adverse reactions in the 2 groups were compared.Results:The total clinical effective rate of the observation group(93.33%)was significantly higher than that of the control group(66.67%)(P<0.05); Compared with those before treatment,36,72,108,and 144 h after treatment,the TcB levels of the 2 groups decreased significantly,and gradually decreased over time,and the observation group was lower than the control group; compared with those before treatment,the abnormal rates of BAEP,aEEG and MRI examinations of the 2 groups significantly reduced after treatment,and the observation group was significantly lower than the control group(P<0.05); compared with that of the control group(16.67%),the total incidence of adverse reactions(30.00%)in the observation group was not statistically significant(P>0.05).Conclusion:Yinzhihuang Oral Liquid combined with Saccharomyces boulardii has a significant effects on neonatal hyperbilirubinemia,which can significantly reduce TcB levels and improve the nervous system damage in child patients,and it was conducive to prognosis and has good safety,which is worthy of clinical application.
