    目的:探讨桂枝茯苓丸治疗子宫肌瘤模型大鼠的作用机制。方法:60只成年雌性SD大鼠随机分为4组:分为空白(Control)组、模型(Hys)组、桂枝茯苓丸低剂量(G-L)组、桂枝茯苓丸高剂量(G-H)组,每组15只。除假手术组外,其余3组均进行子宫肌瘤造模。G-L组、G-H组分别接受浓度为1.32 g/mL、2.64 g/mL的桂枝茯苓丸混悬液1 mL灌胃,其中Control组及Hys组大鼠给予等剂量生理盐水灌胃,1次/d,连续8周。记录各组大鼠子宫及卵巢系数,HE染色观察子宫形态学变化,透射电镜观察子宫超微结构变化,ELISA法检测子宫组织E2、P、ER含量,Western Blot法检测子宫组织中PI3K/Akt信号通路蛋白表达情况。结果:与Control组比较,Hys组的子宫和卵巢系数、E2、P、ER的含量及抗凋亡蛋白PI3K、p-Akt表达增高(P<0.05),促凋亡蛋白Caspase-3表达减少(P<0.05),G-L组、G-H组上述指标均有所改善(P<0.05),但G-H组明显优于G-L组(P<0.05)。HE染色及透射电镜形态学观察提示Hys组大鼠子宫平滑肌增厚,细胞边界不清晰,细胞核固缩呈椭圆形或畸形,胞质内线粒体肿胀,部分嵴断裂消失,可见粗面内质网扩张,肌纤维排列紊乱无序,可见明显炎性细胞浸润;G-L组子宫平滑肌细胞排列较Hys组有序,平滑肌层仍有所增厚,炎性细胞浸润亦较Hys组组减少,细胞核较Hys组稍增大,线粒体肿胀和粗面内质网扩张减少;G-H组子宫平滑肌细胞排列较Hys组明显有序、紧密,仅见轻微炎性细胞浸润,平滑肌层无明显增生,且平滑肌细胞核较正常,线粒体及粗面内质网形态接近正常。结论:桂枝茯苓丸有效改善子宫肌瘤模型大鼠的形态学指标,其机制可能与激活PI3K/AKT通路,下调E2、P、ER的表达,上调Caspase-3的表达有关。

    Abstract:

    To explore the mechanism of Guizhi Fuling Pill in the treatment of uterine leiomyoma model rats.Methods:A total of 60 adult female SD rats were randomly divided into 4 groups:a blank (Control) group (n=15),a model (Hys) group (n=15),a low dose Guizhi Fuling pill (G-L) group (n=15) and a high dose Guizhi Fuling Pill (G-H) group (n=15).Except for the sham operation group,the other 3 groups were modeled by uterine leiomyoma.G-L group and G-H group received 1.32 g/mL and 2.64 g/mL Guizhi Fuling Pill suspension 1 mL respectively,and the rats in the control group and Hys group were given the same dose of normal saline once a day for 8 weeks.The coefficients of uterus and ovary were recorded,the morphological changes of uterus were observed by HE staining,the ultrastructural changes of uterus were observed by transmission electron microscope,the contents of E2,P and ER in uterine tissue were detected by ELISA method,and the expression of PI3K/Akt signal pathway protein in uterine tissue was detected by Western Blot method.Results:Compared with Control group,the coefficient of uterus and ovary,the content of E2,P,ER and the expression of anti-apoptotic protein PI3K and p-Akt increased (P<0.05) and the expression of pro-apoptotic protein Caspase-3 decreased (P<0.05) in Hys group.The above-mentioned indexes were improved in G-L group and G-H group (P<0.05),but the expression of anti-apoptotic protein PI3K and p-Akt in G-H group was significantly better than that in G-L group (P<0.05).The morphological observation of HE staining and transmission electron microscope showed that the uterine smooth muscle of Hys group thickened,the cell boundary was not clear,the nuclear pyknosis was oval or abnormal,the mitochondria in the cytoplasm swelled,part of the crest ruptured and disappeared,the rough endoplasmic reticulum dilated,the muscle fibers were disordered,and obvious inflammatory cell infiltration could be seen.The arrangement of uterine smooth muscle cells in G-Lgroup was more orderly than that in Hys group,the smooth muscle layer was still thicker,the infiltration of inflammatory cells was less than that in Hys group,the nucleus was slightly larger,and the swelling of mitochondria and dilatation of rough endoplasmic reticulum in G-L group were less than those in Hys group.The arrangement of uterine smooth muscle cells in G-H group was more orderly and compact than that in Hys group,only slight inflammatory cell infiltration,no obvious proliferation of smooth muscle layer,normal smooth muscle nucleus,mitochondria and rough endoplasmic reticulum were close to normal.Conclusion:Guizhi Fuling Pill can effectively improve the morphological indexes of uterine leiomyoma model rats,and its mechanism may be related to the activation of PI3K/AKT pathway,down-regulation of E2,P,ER expression and up-regulation of Caspase-3 expression.
