| 目的：基于肠道菌群对生殖激素的影响探讨大黄牡丹汤治疗子宫肌瘤的作用机制。方法：选取2017年1月至2019年1月海南医学院附属第二医院收治的确诊为子宫肌瘤患者96例作为研究对象，按就诊顺序分为对照组和观察组，每组48例。对照组口服米非司酮，观察组加用大黄牡丹汤，均治疗3个月。观察2组生殖激素、肠道菌群、细胞因子、T淋巴细胞亚群、NK细胞、中医证候积分、子宫体积、肌瘤体积变化。结果：1)2组治疗后雌激素（E2）、孕酮（P）、促卵泡激素(FSH)、黄体生成素（LH）、白细胞介素-6（IL-6）、肿瘤坏死因子-2α（TNF-2α）、CD8+、肠杆菌、酵母样真菌均较治疗前下降（均P<0.05），IL-22、CD3+、CD4+、CD8+、CD4+/CD8+、NK细胞、肠球菌、双歧杆菌、乳杆菌等均较治疗前升高（均P<0.05），且观察组均优于对照组（均P<0.05）。2)2组治疗后下腹包块、月经改变、子宫体积、肌瘤体积等均较治疗前下降（均P<0.05），观察组均显著低于对照组（P<0.05）。3)观察组临床有效率高于对照组（P<0.05）。结论：大黄牡丹汤通过调节肠道菌群降低生殖激素水平、提高细胞免疫因子，从而缩小肌瘤体积。 |

| To explore the mechanism of DahuangMudan Decoction in the treatment of uterine leiomyoma based on the effect of intestinal flora on reproductive hormones.Methods:A total of 96 patients with uterine leiomyoma admitted to the Second Affiliated Hospital of Hainan Medical College were selected as the research objects and were randomly divided into a control group(n=48)and an observation group(n=48)according to order of visits.The control group was treated with mifepristone and the observation group was treated with DahuangMudan Decoction for 3 months.The changes of reproductive hormone，intestinal flora，cytokines，T lymphocyte subsets，NK cells，Traditional Chinese medicine syndrome score，uterine volume and myoma volume were observed between the 2 groups.Results:1)after treatment，estrogen(E2)，progesterone(P)，follicle stimulating number(FSH)，luteinizing hormone(LH)，interleukin-6(IL-6)，tumor necrosis factor-2 α(TNF-2 α)，CD8+，enterobacteria and yeast-like fungi decreased，while IL-22，CD3+，CD4+，CD4+/CD8+，NK cells，enterococci，bifidobacteria and lactobacillus increased after treatment.The observation group was better than the control group(P<0.05).2)after treatment，the lower abdominal mass，menstrual changes，uterine volume and myoma volume between the 2 groups were significantly lower than those before treatment(P<0.05)，and those in the observation group were significantly lower than those in the control group(P<0.05).3)The clinical effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion:Dahuang Mudan Decoction can inhibit reproductive hormones and increase cellular immune factors by regulating intestinal flora，and to reduce the volume of leiomyoma. |
