      目的:利用GEO芯片数据及中医传承辅助系统,获取气虚血瘀型脑梗死的差异基因及中医药防治方剂,并分析其作用机制,为气虚血瘀型脑梗死的中医药防治策略提供理论依据。方法:采用生物信息学、数据挖掘、网络药理学等多学科交叉方法,对气虚血瘀型脑梗死的中医药防治策略进行探讨。整合基因表达综合数据库(GEO)、中医药整合药理学研究平台(TCMIP)、GeneCards、PubChem、中药系统药理学数据库与分析平台(TCMSP)、STRING、WebGestalt、DAVID多数据库资源及Cytoscape、R语言软件工具,对气虚血瘀型脑梗死疾病靶点、中药靶点进行筛选,从蛋白质-蛋白质相互作用(PPI)网络、基因本体(GO)及京都基因与基因组百科全书(KEGG)富集的角度,对中医传承辅助系统数据挖掘获取的气虚血瘀型脑梗死基础方的作用机制进行探讨。结果:获取2 542个气虚血瘀型脑梗死疾病靶点,其中差异基因结果显示HIF1A、PIK3CA、PIK3R1、TP53、NFKB1、HSP90AA1、CREBBP、CASP8、IL6、TNF、ITGB等表达上调,APOE、IGF1、CXCL5、SRC、ADCY5、PTGS2等表达下调,可作为疾病诊断治疗的潜在靶点。58首气虚血瘀型脑梗死方剂的用药规律,包括频次、关联及新方组合,获得气虚血瘀型脑梗死基础方剂,由黄芪、川芎、当归、地龙、党参、西洋参、白术、桃仁、赤芍、枳实10味中药组成,具有益气活血、行气化痰的功效,其作用机制可能与调控差异基因以多组分、多功能、多途径的方式,主要通过HIF-1、TNF、NF-kappa B、Apoptosis、PI3K-AKT等通路发挥作用。结论:气虚血瘀型脑梗死的中医药防治当以差异基因靶点为中心,筛选出临床基础方,根据临床辨证施治加减中药,同时可采用多学科交叉方法对气虚血瘀型脑梗死的方剂从多组分、多途径、多靶点层面进行临床与实验验证。本研究为中医药治疗气虚血瘀型脑梗死提供了方法理论上的指导,对推动中医药现代化具有重要意义。
English Summary:
      To use GEO chip data and traditional Chinese medicine inheritance auxiliary system to obtain the differential genes and traditional Chinese medicine prevention and treatment prescriptions of cerebral infarction of qi deficiency and blood stasis type,and to analyze its mechanism,so as to provide theoretical basis for the prevention and treatment strategy of traditional Chinese medicine of qi deficiency and blood stasis type of cerebral infarction.Methods:The prevention and treatment strategy of traditional Chinese medicine for cerebral infarction of qi deficiency and blood stasis was discussed by means of bioinformatics,data mining,network pharmacology and other interdisciplinary methods.Integrate GEO,TCMIP,GeneCards,PubChem,TCMSP,String,WebGestalt,DAVID multi-database resources and Cytoscape,R language software tools to screen disease targets and traditional Chinese medicine targets of qi deficiency and blood stasis type of cerebral infarction.From the point of view of PPI network,GO and KEGG enrichment,the mechanism of the basic side of qi deficiency and blood stasis type of cerebral infarction obtained by data mining of TCM inheritance auxiliary system were discussed.Results:A total of 2542 disease targets of cerebral infarction with qi deficiency and blood stasis were obtained.The results of differential genes showed that the expressions of HIF1A,PIK3CA,PIK3R1,TP53,NFKB1,HSP90AA1,CREBBP,CASP8,IL6,TNF and ITGB were up-regulated,while the expressions of APOE,IGF1,CXCL5,SRC,ADCY5 and PTGS2 were down-regulated,which can be used as potential targets for disease diagnosis and treatment.A total of 58 the medication rule of the first qi deficiency and blood stasis type of cerebral infarction prescription,including frequency,correlation and new prescription combination,to obtain the basic prescription of qi deficiency and blood stasis type of cerebral infarction,which is composed of Astragali Radix,Ligusticum chuanxiong Hort,Angelica sinensis (Oliv.) Diels,Pheretima,Codonopsis Radix,Panacis Quinquefolii Radix,Atractylodes macrocephala Koidz,Persicae Semen,Paeoniae Rubra Radix,Aurantii Immaturus Fructus,which have the effect of replenishing qi and activating blood circulation,promoting qi and resolving phlegm,and its action mechanism may be related to the regulation of differential genes in a multi-component,multi-functional and multi-channel way.It plays a role mainly through HIF-1,TNF,NF-kappaB,Apoptosis,PI3K-AKT and other pathways.Conclusion:The prevention and treatment of cerebral infarction of qi deficiency and blood stasis type by traditional Chinese medicine should focus on the target of different genes,select the clinical basic prescription,and add or subtract traditional Chinese medicine according to clinical syndrome differentiation.At the same time,multi-disciplinary interdisciplinary method can be used to verify the clinical and experimental results of traditional Chinese medicine in the treatment of qi deficiency and blood stasis cerebral infarction from multi-component,multi-pathway and multi-target levels.This study provides theoretical guidance for the treatment of qi deficiency and blood stasis cerebral infarction with traditional Chinese medicine.It is of great significance to promote the modernization of traditional Chinese medicine.
