目的:系统评价中药复方治疗非糜烂性胃食管反流病(NERD)的临床疗效与安全性。方法:计算机检索国家知识基础设施数据库(CNKI)、中国学术期刊数据库(CSPD)、中文科技期刊数据库(CCD)、中国生物医学文献数据库(CBM)、PubMed、Cochrane、EMbase数据库,收集中药复方治疗NERD的随机对照试验(RCT),检索时间范围均为建库至2022年4月。由2名研究者独立筛选文献、提取资料并评价纳入研究质量后,用Review Manager 53软件进行Meta分析,并用Stata SE120检验发表偏倚风险。结果:共纳入17个RCT,包括1 630例患者。Meta分析结果显示:与西药组比较,中药复方组治疗NERD的总有效率高于西药组(RR=116,95%CI为110~123,Z=519,P<0000 1);复发率(RR=031,95%CI为022~045,Z=422,P<0000 1)、不良反应发生率(RR=019,95%CI为009~043,Z=399,P<0000 1)均低于西药组;反流性疾病问卷积分与西药组比较差异无统计学意义(SMD=-068,95%CI为-150~013,Z=164,P=010)。结论:中药复方治疗NERD的总体疗效优于常规西药治疗,且具有复发率低、不良反应少的优势。未来应开展更多高质量研究以进一步验证上述结论。

English Summary:

To systematically evaluate the clinical efficacy of Chinese medicine compound prescriptions in the treatment of non-erosive reflux disease(NERD).Methods:The papers about the randomized controlled trials(RCTs) of Chinese medicine compound prescriptions in the treatment of NERD were retrieved from China Biology Medicine(CBM)，Chinese National Knowledge Infrastructure(CNKI)，Chinese Citation Database(CCD)，China Science Periodical Database(CSPD)，PubMed，Cochrane，and EMbase，with the time interval from database inception to April 2022.After two researchers independently screened the literature，extract data，and evaluate the quality of included studies，meta-analysis was conducted in Review Manager 53 and the risks of publication bias were tested by Stata SE120.Results:A total of 17 RCTs were included，involving 1 630 patients.The meta-analysis showed that the Chinese medicine compound prescription group had higher total response rate(RR=116，95%CI of 110 to 123，Z=519，P<0000 1)，lower recurrence rate(RR=031，95%CI of 022 to 045，Z=422，P<0000 1)，and lower incidence of adverse reactions(RR=019，95%CI of 009 to 043，Z=399，P<0000 1) than the western medicine group in treating NERD.RDQ score showed no significant difference between groups(SMD=-068，95%CI of -150 to 013，Z=164，P=010).Conclusion:Chinese medicine compound prescriptions outperform conventional western medicine in the treatment of NERD and have the advantages of lower recurrence rate and fewer adverse reactions.
