目的:评价血府逐瘀汤对心血管疾病相关炎症介质的影响。方法:通过Pubmed、Embase、Cochrane Library、国家知识基础设施数据库(CNKI)、中国学术期刊数据库(CSPD)及中文科技期刊数据库(CCD)进行文献检索,应用主题词结合自由词的方式制定检索策略,获取血府逐瘀汤对心血管疾病炎症介质影响的随机对照试验(RCT),根据Cochrane手册对纳入的RCT进行文献质量评价,整理并提取相关数据资料,应用RevMan 5.3软件进行Meta分析。结果:共纳入23项RCT,包括2 274例患者。Meta分析结果显示,血府逐瘀汤可显著降低炎症介质C反应蛋白(SMD=-2.04,95%CI为-2.82~-1.26,P<0.000 01)、超敏C反应蛋白(SMD=-1.46,95%CI为-1.96~-0.96,P<0.000 01)、肿瘤坏死因子-α(SMD=-1.25,95%CI为-1.68~-0.83,P<0.000 01)和白细胞介素-6(SMD=-1.08,95%CI为-1.45~-0.70,P=0.003)的水平。结论:血府逐瘀汤可通过降低炎症介质的表达,控制炎症反应,进而改善心血管疾病症状,但该结论的准确性受到纳入文献质量的影响,有待高质量的RCT提供更加可靠的循证医学证据。 |

To evaluate the effects of Xuefu Zhuyu Decoction on inflammatory factors in cardiovascular diseases.Methods:Pubmed,Embase,Cochrane Library,CNKI,CSPD,and CCD were searched for randomized controlled trials(RCTs) of Xuefu Zhuyu Decoction against inflammatory factors in cardiovascular diseases with the retrieval strategy of the combination of subject terms and free terms.The quality of included RCTs was assessed by the Cochrane Handbook for Systematic Reviews of Interventions.The data were extracted,followed by Meta-analysis with Revman5.3 software.Results:Twenty-three RCTs were included,involving 2 274 cases.Meta-analysis showed that Xuefu Zhuyu Decoction could significantly reduce the levels of inflammatory factors such as CRP(SMD=-2.04,95%CI -2.82 to -1.26,P<0.000 01),hs-CRP(SMD=-1.46,95%CI -1.9 to -0.96,P<0.000 01),TNF-α(SMD=-1.25,95%CI -1.68 to -0.83,P<0.000 01),and IL-6(SMD=-1.08,95%CI -1.45 to -0.70,P=0.003).Conclusion:Xuefu Zhuyu Decoction could reduce the expression of inflammatory factors and control inflammatory responses to improve the symptoms of cardiovascular diseases.Since the accuracy of this conclusion was limited by the quality of the included research articles,more high-quality RCTs are needed to provide reliable evidence-based medicine support. |
