      目的:探讨麦粒灸合补肺祛瘀方对肺纤维化(PF)大鼠肺功能及核因子κB信号通路的影响。方法:选取60只成年SD大鼠(雌雄各半)按随机数字表法均分为空白对照组、模型组、泼尼松龙组、麦粒灸组、方药组、灸药组,每组10只。气管内注射博来霉素制备PF模型,泼尼松龙组灌胃给予泼尼松龙(5 mg/kg)、麦粒灸组进行麦粒灸、方药组给予补肺祛瘀方灌胃、灸药组同时进行麦粒灸和灌胃补肺祛瘀方(5 g/kg)空白对照组和模型组灌胃等体积生理盐水。30 d后检测大鼠肺功能,对PF大鼠肺组织进行病理学检测,同时检测支气管肺泡灌洗中肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和白细胞介素-6(IL-6)水平及肺组织中核因子κB、转化生长因子-β(TGF-β)和Smad2 mRNA水平。结果:空白对照组大鼠肺组织未见明显异常;模型组肺组织中胶原沉积明显,炎症细胞浸润,肺泡壁增厚,肺泡破坏和紊乱以及肺泡间隙部分塌陷;经泼尼松龙、麦粒灸和方药治疗后,肺组织胶原沉积明显减少,炎症和纤维化明显改善,其中泼尼松龙组改善最显著。与空白对照组比较,其余组大鼠用力肺活量(FVC)、最大通气量(MVV)和动态顺应性(Cdyn)降低,TNF-α、IL-1β、IL-6、核因子κB、TGF-β和Smad2升高(P<0.05);与模型组比较,各治疗大鼠FVC、MVV和Cdyn升高,TNF-α、IL-1β、IL-6、核因子κB、TGF-β和Smad2降低,其中灸药组疗效好于麦粒灸组和方药组,而泼尼松龙组疗效最好(P<0.05)。结论:麦粒灸合补肺祛瘀方可以改善PF大鼠肺功能,其作用机制可能与核因子κB信号通路抑制有关。
English Summary:
      To investigate the effect of moxibustion with seed-sized moxa cone combined with Bufei Quyu Recipe(BQR) on lung function and nuclear factor kappa-B(NF-κB) signaling pathway in rats with pulmonary fibrosis(PF).Methods:A total of 60 adult rats(SD,half males and half females) were selected and divided into blank group,model group,prednisolone group,moxibustion group,BQR group and moxibustion+BQR group according to random number table,with 10 in each group.PF model was established by intratracheal injection of bleomycin.Prednisolone group was treated with prednisolone(5 mg/kg),and moxibustion group was given moxibustion with seed-sized moxa cone.BQR group was treated with BQR(5 g/kg),and moxibustion+BQR group received moxibustion with seed-sized moxa cone and BQR simultaneously.The blank group and model group were given equal volume of normal saline as control.After 30 days of treatment,the lung function of the PF rats was detected,and the lung tissue was subject to pathological examination.Additionally,the levels of tumour necrosis factor α(TNF-α),interleukin-1β(IL-1β) and IL-6 in bronchoalveolar lavage fluid(BALF) and the mRNA levels of NF-κB,TGF-β and Smad2 in lung tissue were determined.Results:In the blank group,there was no obvious abnormality in the lung tissue.In the model group,marked collagen deposition,inflammatory cell infiltration,alveolar wall thickening,alveolar destruction and disorder and partial collapse of alveolar space were observed.After treatment with prednisolone,moxibustion with seed-sized moxa cone and BQR,collagen deposition in the lung tissue was significantly reduced and inflammation and fibrosis were significantly improved,with the most significant improvement in prednisolone group.Compared with the conditions in blank group,the forced vital capacity(FVC),maximum voluntary ventilation(MVV) and dynamic compliance(Cdyn) in the other groups were decreased,and TNF-α,IL-1β,IL-6,NF-κB,TGF-β and Smad2 were increased(P<0.05).Compared with the model group,the treatment groups presented elevated FVC,MVV and Cdyn and reduced TNF-α,IL-1β,IL-6,NF-κB,TGF-β and Smad2.The curative effect of moxibustion+BQR group was better than that of moxibustion group and BQR group,while prednisolone group had the optimal curative effect(P<0.05).Conclusion:Moxibustion with seed-sized moxa cone combined with BQR can improve the lung function of PF rats,and its mechanism may be related to the inhibition of NF-κB signaling pathway.
