| 急性心肌梗死（AMI）是以冠状动脉粥样硬化、斑块破裂及血栓形成为特征的急性冠状动脉闭塞症，其高发病率、高致死率给社会及家庭带来严重的影响。气虚血瘀证是AMI的常见证型，益气活血是主要的治疗原则，补阳还五汤、芪参益气滴丸、参松养心胶囊等是常用方。目前对AMI气虚血瘀证的临床表现和发病机制研究较多，益气活血方对AMI的治疗具有多层次、多成分、多靶点的整体优势，可促进血管再生、抑制心室重构、保护血管内皮、减轻氧化应激和炎症反应、增加冠脉血流、抑制细胞凋亡、改善中医证候等，在心血管疾病的防治方面得到广泛应用。同时，益气活血方可通过调控微RNA（miRNAs）表达，从而调节新生血管形成、心肌细胞凋亡、炎症反应及减轻心室重构等方面发挥治疗AMI的作用，进一步明确中医药的靶向性。中医学对AMI的病因病机及治疗有独特的认识，现对近5年来益气活血方治疗AMI及与相关miRNAs的作用机制进行综述。 |

| Acute myocardial infarction(AMI) is an acute coronary artery occlusion characterized by coronary atherosclerosis，plaque rupture，and thrombosis.It has high morbidity and mortality，posing serious impacts on the society and families.AMI patients mainly present the syndrome of qi deficiency and blood stasis.Therefore，replenishing qi and activating blood are the main therapeutic principles for AMI.Buyang Huanwu Decoction，Qishen Yiqi Pills，and Shensong Yangxin Capsules are commonly used prescriptions.Studies have been conducted concerning the clinical symptoms and pathogenesis of the AMI patients with the syndrome of qi deficiency and blood stasis.Yiqi Huoxue Decoction can treat this disease in a multi-component，multi-level，and multi-target manner.It can promote angiogenesis，inhibit ventricular remodeling and cell apoptosis，protect vascular endothelium，reduce oxidative stress and inflammatory response，boost coronary blood flow，and alleviate traditional Chinese medicine(TCM) syndromes.Therefore，this prescription is widely used in the prevention and treatment of cardiovascular diseases.Moreover，by regulating the expression of related miRNAs，Yiqi Huoxue Decoction can treat AMI via several mechanisms，including regulating angiogenesis，myocardial cell apoptosis，and inflammatory response and reducing ventricular remodeling.Making clear its role in regulating the expression of these miRNAs helps to clarify the targeting mechanisms of TCM，which has a unique theory about the etiology，pathogenesis，and treatment of AMI.This paper reviews the progress in the mechanisms of Yiqi Huoxue Decoction in the treatment of AMI and related miRNAs in the last 5 years. |
