      目的:从组织学观察三七总皂苷(PNS)对大鼠跟腱异位骨化(HO)的影响,并基于转化生长因子-1(TGF-1)/Sma和Mad相关蛋白(Smad)同源物2/3信号通路探讨其潜在作用机制。方法:将126只SD大鼠随机分为正常对照组、假手术组、模型组、西药组、高剂量组、中剂量组、低剂量组,每组18只。HO造模术后分别给予不同浓度PNS或对照西药或生理盐水灌胃2周。于术后2周、5周及10周分别行小动物X线及CT平扫评估HO发生情况。采用一般组织学染色,包括苏木精-伊红染色、Masson三色染色及番红O固绿软骨染色,观测HO病灶内组织学改变。经免疫组织化学法比较各组病灶局部TGF-β1、磷酸化Smad同源物2(p-Smad2)及p-Smad3表达的差异。结果:术后10周,X线检测可见各手术大鼠成骨率差异无统计学意义(P>0.05)。术后2周,手术大鼠跟腱细胞显著增加(P<0.05),跟腱纤维排列紊乱;5周可见大量增殖肥大软骨细胞;10周可见大量成熟异位骨组织形成。中剂量组术后10周相对骨量显著低于模型组(P<0.05)。免疫组织化学染色显示,假手术组TGF-β1、p-Smad2及p-Smad3表达术后与正常对照组比较均显著增加,差异均有统计学意义(均P<0.01)。与模型组比较,中剂量组TGF-β1的表达在术后2周显著降低(P<0.05),表达峰值延后。结论:TGF-β1的表达在大鼠跟腱HO造模术后具有显著增加。一定浓度的PNS可降低异位骨组织的相对骨量,这可能与PNS延后TGF-β1的表达峰值并下调其表达量有关。
English Summary:
      To determine the histological effect of Panax notoginseng saponin(PNS) on heterotopic ossification(HO) of Achilles tendon in rats and identify the underlying mechanism based on transforming growth factor-β1(TGF-β1)/Sma-and Mad-related protein(Smad) 2/3 signaling pathways.Methods:A total of 126 Sprague-Dawley(SD) rats were randomly divided into seven groups:a normal control group,a sham operation group,a model group,a western medicine treatment group,a high dose group,a medium dose group,and a low dose group,with 18 rats in each group.After receiving HO modeling surgeries,rats were treated with PNS in different concentrations,western medicine,or normal saline by gavage for two weeks.The incidence of HO was evaluated by X-ray and CT scan at 2,5,and 10 weeks postoperatively.A series of general histological staining methods,including hematoxylin-eosin(HE) staining,Masson's trichrome staining,and Safranin O-fast green staining,were used to evaluate the histological changes in HO lesions.The expression of TGF-β1,phosphorylated Smad 2(p-Smad2),and p-Smad3 in the local lesion was compared among groups by immunohistochemistry.Results:At 10 weeks postoperatively,results of the X-ray examination showed that there was no significant difference in the rate of bone formation among all groups receiving HO modeling surgeries(P>0.05).At two weeks after surgery,there was a significant increase in Achilles tendon cells in rats(P<0.05) and a disturbed arrangement of Achilles tendon fibers.A large number of proliferating hypertrophic chondrocytes were observed at five weeks and a large amount of mature ectopic bone tissues was formed at 10 weeks.The relative bone mass at 10 weeks in the medium dose group was significantly lower than that in the model group(P<0.05).The results of immunohistochemistry showed that the postoperative expression of TGF-β1,p-Smad2,and p-Smad3 in the sham operation group was significantly higher than that in the normal control group(P<0.01).Compared with the model group,the medium dose group exhibited significantly decreased TGF-β1 expression two weeks after surgery(P<0.05) with delayed peak expression.Conclusion:The expression of TGF-β1 increased significantly after the HO modeling surgery of the Achilles tendon in rats.A certain concentration of PNS can reduce the relative bone mass of ectopic bone tissues,which may be related to the possibility that PNS delayed the peak expression of TGF-β1 and down-regulated its expression.
