To explore the clinical experience of experts in the application of Suhexiang Pills for the treatment of stroke through a questionnaire survey，identify the target population and assess the safety of Suhexiang Pills，and facilitate the design and optimization of the next phase of real-world research protocols.Methods:A nationwide questionnaire survey targeting clinical physicians was conducted from August 2021 to October 2021，and the survey results were statistically analyzed.Results:A total of 231 questionnaires were collected，and the interviewed experts were considered representative and authoritative.Clinical issues of concern to experts included indications，contraindications，and applicable stages for Suhexiang Pills.In terms of traditional Chinese medicine(TCM) indications，the majority of experts believed that Suhexiang Pills were clinically applicable for stroke with wind striking meridian and collateral syndrome(82.3%) and yin blockage in organ stroke syndrome(70.6%).In terms of western medicine indications，experts generally agreed that Suhexiang Pills were suitable for both ischemic stroke patients(97.4%) and hemorrhagic stroke patients(93.9%).Regarding the timing and duration of intervention，61.0% of experts indicated that Suhexiang Pills could be used within 12 hours after onset，and the optimal treatment duration was less than 14 days(74%).In terms of safety，pregnancy(79.2%) and allergies(78.8%) were identified as the main contraindications for Suhexiang Pills.There were potential risks of liver dysfunction(61.0%) and kidney dysfunction(59.3%) associated with Suhexiang Pills.Conclusion:Experts generally believe that Suhexiang Pills are clinically suitable for the treatment of ischemic and hemorrhagic stroke patients who present with clinical manifestations such as clouding of consciousness，dizziness，vertigo，and phlegm-rattling in the throat caused by phlegm-dampness obstructing the orifices，and can be used in various clinical stages of stroke，mainly focusing on the acute phase.Suhexiang Pills are characterized by depleting qi with aromatics，so they should not be used for prolonged periods.Pregnancy and allergies are contraindications for Suhexiang Pills.Additionally，the liver and kidney toxicities caused by Cinnabaris should be carefully considered in clinical applications.
