| 目的：观察补阳还五汤联合针灸治疗糖尿病周围血管病（DPVD）的疗效。方法：选取2019年6月至2022年6月北京中医医院怀柔医院收治的气虚血瘀证DPVD患者96例作为研究对象，以随机数表法分成观察组与对照组，每组48例。对照组给予针灸治疗，观察组在此基础上给予补阳还五汤治疗。比较2组患者临床疗效，治疗前后临床证候评分，踝肱指数（ABI）及足背动脉血流，血糖代谢空腹血糖（FBG）、餐后2 h血糖（2 h PG）、糖化血红蛋白（HbA1c），血流变学低切全血黏度（BVL）、高切全血黏度（BVH）、血浆黏度（PV）、纤维蛋白原（Fg）水平，以及治疗期间不良反应的发生情况。结果：治疗后观察组疗效总有效率为91.67%，高于对照组的68.75%，且观察组各临床证候评分低于对照组（P<0.05），同时ABI与足背动脉流血量高于对照组（P<0.05）。此外治疗后观察组患者FBG、2 h PG、HbA1c、BVL、BVH、PV及Fg水平低于对照组（均P<0.05）。2组不良反应总发生率比较，差异无统计学意义（P>0.05）。结论：补阳还五汤联合针灸治疗气虚血瘀证DPVD患者临床疗效确切，能缓解临床症状与周围血管病变情况，并有助于改善血糖代谢及血流变学水平。 |

| To observe the efficacy of Buyang Huanwu Decoction combined with acupuncture and moxibustion in treating diabetes peripheral vascular disease(DPVD).Methods:A total of 96 DPVD patients with qi deficiency and blood stasis syndrome from June 2019 to June 2022 at Huairou Hospital of Beijing Hospital of Traditional Chinese Medicine were enrolled and randomly divided into an observation group(n=48) and a control group(n=48) using a random number table.The control group received acupuncture and moxibustion treatment，while the observation group received Buyang Huanwu Decoction in addition to acupuncture and moxibustion treatment.The clinical efficacy，clinical syndrome scores before and after treatment，ankle-brachial index(ABI)，foot dorsalis artery blood flow，fasting blood glucose(FBG)，2-hour postprandial blood glucose(2h PG)，glycated hemoglobin(HbA1c)，hemorheological parameters [low-shear whole blood viscosity(BVL)，high-shear whole blood viscosity(BVH)，plasma viscosity(PV)，and fibrinogen(Fg)]，and the occurrence of adverse reactions during the treatment were compared between the two groups.Results:The total effective rate of the observation group after treatment was 91.67%，which was higher than 68.75% in the control group.The clinical syndrome score in the observation group was lower than that in the control group(P<0.05)，and ABI and foot dorsalis artery blood flow were higher than those in the control group(P<0.05).Additionally，after treatment，the levels of FBG，2h PG，HbA1c，BVL，BVH，PV，and Fg in the observation group were lower than those in the control group(all P<0.05).The total incidence of adverse reactions between the two groups did not show a statistically significant difference(P>0.05).Conclusion:Buyang Huanwu Decoction combined with acupuncture and moxibustion treatment for DPVD patients with qi deficiency and blood stasis syndrome showed significant clinical efficacy，which can alleviate clinical symptoms and peripheral vascular lesions，and improve blood glucose metabolism and hemorheological parameters. |
